1.The healthcare professionals should meet to discuss the treatment plan for Mr.
Yoshiama and consider his cultural practices and beliefs and to incoporate this into his
treatment plan. If it is typical for his wife to do certain things for him at home, the team
may want to start to work with both the patient and his wife to reach certain care goals.
Another option may be to discuss as a team how to approach the patient and his wife
while repsecting their culture, but still presenting to them that the goal is to help Mr.
Yoshiama become more independent after his stroke. However the team decides to
approach this scenario, it is essential that they do this as a collaborative effort. This will
assist them in helping the patients they serve to reach their treatment goals.
2.The staff can demonstrate empathy for Mr. Yoshiama and his wife and still respect
their culture and beliefs in a few different ways. One example is to involve Mr. Yoshiama and
his wife in the decision making process. One method to achieve this is to develop goals
together and the determine how they can achieve those goals. If the therapy team is not
involving the patient and his wife in these decisions they may not be the goals the patient
wishes to achieve. Another example is to facilitate effective interventions by understanding
the patient as a whole. In this scenario, it is important to better understand the patient’s culture
and to respect his culture, while also helping him to advance in his therapy.
3.In this scenario there are many differences between the physical therapy team and
the patient and his wife. An example is the team is trying to assist the patient to be more
independent, however, when the patient’s wife is present, he tells her to do these tasks for him
and she does as this is part of their culture. The healthcare team will use strategies to develop
cultural competence. These strategies are understanding general cultural. The healthcare team

needs to understand the couples cultural differences, but they need to do some research on it to
see how they can work through differences. Another one would be accepting and respecting
cultural differences by the team accepting his wife does those task for him and respecting it.
Lastly, apply cultural understanding. The team and the couple need to discuss methods of how
they can do therapy with involing their culter.
4. The ethical dilemma in this scenario is the therapy team has set goals for Mr.
Yoshiama to achieve that do not alighn with his cultural practices. The team is trying tohelp
him become more independent, but in his culture, his wife assists him with many ofhis
activities of daily living. The team sees this as the patient not making progress and there are
certain things that he has to do in order to remain in therapy. If he isn’t meeting the criteria for
therapy to continue, he may have to stop and the team knows that he is
not able to complete
the tasks by himself. The dilemma for the team is to try to assist Mr. Yoshiama to become more
independent while not offending him or his wife and while keeping their culture in mind.
5.At some point a therapist is going to have an ethical dilemma with a patient. It is
important to recognize that there is a dilemma and then develop a plan to work through the
dilemma. This can be accomplished by using the realm-individual process-situation (RIPS)
model. In this model there are four steps to work through a dilemma. The first step is to
investigate the situation. The second step is to reflect and weigh each aspect of the the
situation. This will help to determine where the biggest impact on the decision is. Next, it is
important to make a decision on what is right and what is wrong. And the final step is to
implement the plan. After the plan is implemented, the therapist must evaluate the
effectiveness and reassess for any changes. It can be very challenging to work through this
process, but it is necessary in order to help the patient to achieve their goals.
