1.Why are four antineoplastic drugs used for Zack?
- The four antineoplastic drugs that Zack used is cyclophophamide, carboplatin, vincristine, and etoposide. He is receiving all of those medications because he has a malignant brain tumor that will require multiple chemotherapy medication and approach for his treatment. Antineoplastic drugs has many adverse effects as well.
2.What is the pharmacologic classification of each of the drugs used?
- These are Zack’s chemotherapy regimen classification

cyclophosphamide: alylating agent, nitrogen mustard, disease modifying antirheumatic drug, and immunosuppresant

carboplatin: platinum-containing agent
3.
vincristine: vinca alkaloid, mitotic inhibitor, and natural product
4.
etoposide: topoisomerase inhibitor
3.What overlapping toxicities might Zack experience with these drugs? Considering his physical assessment findings from today, what system toxicities would be of most concerns?
- The overlapping toxicities that Zack might experience with these drugs is nausea, vomitting, peripheral neuropathy, myelosuppression, and neurotoxicity. Based on Zack’s physical assessment findings from today, the most concern on the system toxicities is myelosuppression because of the decreased WBC count and diminished patellar deep tendon reflexes.
4.Fifteen days after the start of the second cycle of chemotherapy, Zack experiences his nadir. At this time his WBC count is 340/mm; Hgb 9.3 g/dL; Hct 25.7%; and the platelet count is 55,000. Zack has been receiving filgrastim (Neupogen) subcutaneously since discharge and will continue until the WBC count begins to rise. Considering these laboratory values, what would be of most concern to the oncology team?
- Given the laboratory values, the most concern for the oncology team would the low platelet count of 55,000. This could potentially put Zack at risk for bleeding and careful monitoring and possible intervention like platelet transfusions to raise his platelet count. 5.Because of the above concerns, what teaching should Zack and his family receive?
- The teaching that Zack and his family should receive is knowing the potential side effects that can occur when receiving treatment from the antineoplastic medications listed above such as anorexia, nausea, vomiting, weight loss, mouth sores, jaundice, and alopecia. Also knowing the contraindications like severe myelosuppression since he has low platelet and WBC count and how to manage and decrease the risk of reducing adverse effects. They should be educated on what to expect when going through the chemotherapy treatment and the importance of finishing it. The most important thing they also need to know is the resources that is available to them for support such as physical, mental, emotional health support because going through something like this isn’t easy and may feel overwhelming.
