Challenge of Interoperability within the Nationwide Healthcare Framework
Interoperability symbolizes the underlying capability for more than one framework to
interchange healthcare Big Data and in turn utilize the pertinent data after being attained. There
are various forms of issues faced in extending comprehensive and robust interoperability
capabilities within national healthcare frameworks (Torab-Miandoab et al., 2023). One of the
issues stems from a lack of uniformity of exchanged healthcare information coupled with no or
reduced efficacy for the standardization of the involved healthcare Big Data framework. The
challenge arises from data being kept in different and unaligned database sources that may
integrate non-uniform formats that are not harmonious in nature (Torab-Miandoab et al., 2023).
Another issue arises from possible healthcare Big Data confidentiality, security and
privacy challenges such as malware and a lack of adherence to the underlined Health Insurance
Portability and Accountability Act (HIPAA) (Torab-Miandoab et al., 2023). These aspects tend to
hamper the goal of steering extensive fortification of individual patients and patient populations'
personal health information. Another issue is facilitated by reduced or no uniform
communications standards within the confines of Electronic Healthcare Records (EHRs) that can
be used within the national healthcare framework (Torab-Miandoab et al., 2023). The context
reduces the attainability of a harmonized communication framework that is significant for data
exchange, acquisition, analysis, curation and evaluation.

The Centers for Medicare and Medicaid Services (CMS) integrated the Medicare
Promoting Interoperability Program for steering interoperability attainability on a nationwide
scale. The program is intended for implementation by eligible professionals (EPs), qualified
healthcare settings, and Critical Access Hospitals (CAHs) (CMS.gov, 2023). The context is for
fostering, enacting, making innovations and exerting noteworthy utilization of Certified
Electronic Health Record Technology (CEHRT). There is also the mandated requirement for the
underlined eligible entities to make reporting for various forms of score objectives or goals for
the CEHRT (CMS.gov, 2023).
The score metrics include electronic prescribing, healthcare information interchanges,
provider-to-patient interchanges, and population healthcare and clinical health information
interchanges. Additionally, there is a necessity for certifying Security Risk Analysis standards
(CMS.gov, 2023). Another context is the need for certifying Safety Assurance Factors for
Electronic Healthcare Records Resilience (SAFER) protocols and reduction or thwarting of
compatibility coupled with interoperability CERT certification. Another aspect is the necessity
for certifying adherence to the Office of the National Coordinator for Health Information
Technology (ONC) outright reviewing (CMS.gov, 2023).
