Discuss the hierarchy of evidence, specifically as it relates to your literature review. What level of evidence are you finding most abundant? What level of evidence is most difficult to find? Why do you think this is? The hierarchy of evidence is a method in determining the best quality of evidence-based literature to the least quality of evidence-based literature. The hierarchy of evidence comes in a variety of formats with the 7-level format the most common. The seven levels comprise of the following from the base of the lowest quality to the tip of the highest quality: base level, case-controlled, cohort studies, randomized controlled trials (RCTs), critically appraised articles, critically appraised topics, and systematic reviews. The bottom four levels are considered unfiltered with the top three being that of filtered. Filtered is a term that indicates literature that has been critically reviewed by experts and other professional peers within the respective scientific community. The base level contains information that is specific and can be found in text books and other reference books from experts providing their opinions. The next three levels are a little more generalized and contain information from studies that contain patient conditions from specific groups and health conditions and have a more in-depth analysis. The top three levels are filtered and consist of critically reviewed articles, topics, and systematic reviews. The articles are concentrated on a single study. The topics consist of articles and studies that concentrate on a topic but applies to a broader group. The systematic review consists of analyzed reviews and evaluates all available information on a specific topic. The level of evidence I am finding most abundant seem to be the top three levels. There is a combination of literature reviews that consist of articles and topic systematic reviews that do not concentrate on any particular patient groups or groups in general. In my initial search, unfiltered evidence of levels 2-4 seems to be more difficult to find including my specific PICOT question for Arizona or other close geographical areas. I believe levels 2-4
are more difficult to find as these topics are more specific and concentrated within a certain field or patient group. Completing new or specific single studies within a patient population or geographical region may be more time consuming and costly to complete as the information may be new to the general topic of study.
