If I could have a do-over, I would have done this instead…… Discussion Discuss an encounter that was particularly interesting and what you learned from it? This week at clinical I was fortunate this week to have a 66 y/o patient who had a scheduled women’s health exam that we could not do because she had a prolapse blader that was protruding her vagina and obviously blocking the cervix. Surprisingly, she said it doesn’t really bother her and she thought we could push it up and look at the cervix. The patient said she was unaware she and had a prolapse bladder. It was not until a year ago, during a pelvic exam the doctor told her about it. She reports being able to push it up if it’s becomes bothersome. She reports being able to urinate without difficulty, but experiences some incontinent when she cough. I placed a referred for her to see an urogynecology specialist for further evaluation. I learned that there is a specialty for such condition “urogynecology” Looking Ahead What clinical goal would you like to tackle next week ? What is your proposed clinical schedule (date/time/total hours) for next week ? Discussion Improve PHI documentation in EHR. Schedule 02/19-2024 Time 0700- 1800, 02/20/2024 I am going to email/message you to further discuss … (Optional) Reviewing a challenging patient encounter.
Challenges/concerns with my preceptor and/or my clinical site. The absence of the application of EBP/CPGs in the clinical setting. Challenges with gaining access to enough patients in the required age range. Challenges with gaining access to a wide variety of diagnoses for the course population.
