Interactive complexity, or code 90785, is an add-on code that should only be used when certain communication issues make it more difficult to provide psychotherapy or a diagnostic mental evaluation. Some people need to understand the code's intent, while others need to report it because of the interactive complexity of other services, such evaluation and management (E/M) services. This rating indicates more intensity rather than more time when complications or communication problems occur. In addition to primary service codes, it must be utilized. For physicians to bill the interactive difficulty code as an add-on to the psychiatric procedure, at least one of the complicating variables listed in the CPT manual must apply. Minors, caregivers, interpreters, agencies (DFCS, SSI), family members involved, and communication difficulties (anxious, furious, not disclosing, sobbing, etc.) are some of these variables.
