Maritime Investigation Summary Report Title Number: 0001 Maritime Investigation Summary Report Date & Time of Incident: 11/4/13 approximately 1300 hours Date & Time of Start of Investigation: Incident Meets Regulatory Requirement for PSM Investigation: Yes No On 11/4/13, at approximately 1300 hours, on mile marker Ohio River 396, a mate from Sharon Marine Company on the Motor Vessel EKU placed a ladder from the bottom deck of the barge she was standing on to the top deck of an adjacent barge. The vessel was moored at the time of the incident. The weather was clear, and visibility was good for about 2 miles. The air temperature was about 90 degrees with a wind speed of 1-2 MPH, and the river current was about 1-2MPH. On completion of the ladder being in place, the mate began to ascend to the top of the barge without the ladder being tied off or someone holding it per company policy XYZ003-9.5.2. As she made the ascent, the bottom of the ladder slipped from underneath her. The mate fell on the deck and landed on her right rib and right elbow. The mate reported the incident to the captain, and external medical treatment was sought at Shaw Medical Center in Mt. Vernon, Indiana 12345. The address is 456 Street. There was one witness, Karis Chipmunk. The attending physical was I.M.A. Good doctor. The X-rays showed there were two fractures to the ribs. One fracture was to the 3 rd rib, and the other to the 4 th rib. In addition, the elbow was broken. The doctor's report states that no surgery is currently necessary. The mate received 30 days of restricted duty and prescription medication. This is an OSHA recordable and a USCG reportable due to receiving restricted duty prescription medication. Immediate Corrective Actions: All ladders were inspected Incident Causes and Recommended Corrective Actions: Causal Factors Root Causes Recommendations Responsible Party Per XYZ003-9.5.2. Ladders must be tied off, or someone must hold the ladder before descending or ascending the ladder. The ladder was not tied off properly. 1. Tie off the ladder properly. 2.
