Heparin should be discontinued if thrombocytopenia develops. PTT needs monitored regularly and should be 1 ½ to 2 times the normal control values. 8. (7) How you will evaluate the effectiveness of the medication Draw blood for PTT 4 to 6 hours after starting infusion or subcut dose, should be 1.5 to 2x normal control values. 9. (8) How you will educate the client BEFORE administering this medication Report signs of bleeding such as bleeding gums, brusing on the arms or legs, petechiae, nosebleeds, or blood in the urine or stool. Avoid taking OTC drugs containing aspirin. Alert your dentist and other medical personal that you are taking heparin before undergoing any procedures. 1. (4) Safe dose range No more than 50 mg in 10 minutes 5. (5) Acceptable routes and rates (if applicable) 1 mg neutralized 100 USP units of heparin 6. (6) Any critical assessment data (physical, labs, other medications, procedurally, interactions) you need to evaluate prior to giving this medication Given based off coagulation labs (PTT, INR) and when bleeding is present from heparin overdose. 7. (7) How you will evaluate the effectiveness of the medication Reversal of heparin overdose. Coagulation of blood, decrease in bleeding. 8. (8) How you will educate the client BEFORE administering this medication Protamine sulfate can cause severe low blood pressure and anaplylacatoid reactions. Report signs of sensitivity such as difficulty breathing and worsening bleeding.
