Program Title and Program Goal I have decided to title my program “The Crisis Surrounding Opioid Abuse,” as it addresses the issue within my program. The title will also attract individuals with opioid substance abuse and those looking to assist their loved ones with this abuse. Lastly, the location and target population focused on within this program gas and is currently experiencing an opioid crisis. My program's overall goal is to mitigate the opioid epidemic by decreasing the total number of deaths caused by opioid misuse. That said, this goal addresses the identified problem of Cabell County, West Virgina having the highest number of mortalities caused by overdoses in the United States (American Health-Johns Hopkins, n.d.). Short-Term, Intermediate, and Long-Term Outcomes Short term outcomes of this program include meeting with the care coordinators within the first 2-4 weeks of program implementation to address overall health conditions and opioid relevant health outcomes such as bloodborne infections, i.e., hepatitis C and HIV. Other short-term outcomes include reducing cravings, relieve withdrawals, and reducing the rate of opioid relapses. Meeting the care coordinator helps establish a baseline of the individual's health, which lets us determine how opioids contribute to their decline. Intermediate outcomes will consist of and be the result of meeting with care coordinators i.e.; improvement of overall health conditions and management of opioid relevant health outcomes. Monitoring signs of cravings, withdrawals, and opioid relapses, and enrollment to AA meetings for opioid addiction will also be an intermediate outcome. Research shows that
amongst those with substance abuse 40-60% relapse within 30 days of trying to get clean (Sternlicht & Sternlicht, 2023). Long-term outcomes will require a longer time of 4-6 months to determine the program results. Those outcomes include improvement to overall lifestyle i.e.; home life, personal life, and career and decrease the overall rate of opioid crisis. Short-term outcomes will assist in achieving long-term outcomes as the outcomes pinpoint the root issues needed to overcome the opioid crisis. Activities or Work Products Activites for the program would include AA programs, daily community meeting where pharmacists will educate the community on safe usage of prescribed opioids, and motivational speeches from those who have overcame this addiction. AA programs and motivational speeches create a safe space for the community and network of support; this is vital to the environment of the program as it allows individuals to feel comfortable enough to take the steps to overcome this addiction. Pharmacists can play a vital role in referral and education on resources, treatment services, harm reduction, and clinical management of medications for opioid use disorders (Kosobuski et al., 2022). That said, harm reduction resources will include work products such as naloxone, and fentanyl strip tests which are instrumental in preventing overdoses. Nalmefene Hydrochloride (HCl) would be the last work product utilized in the program. Nalmefene Hydrochloride (HCl) is an additional opioid overdose reversal option that is known for its longer duration of action in fully reversing doses verses Naloxone (Purdue Pharma, 2022). To determine the inputs and resources needed for my program we will take into consideration the overall goal and expected outcomes. The resources must be established by the importance of funding and the program's success. The inputs and resources for my program include paid partnerships with AA programs, partnerships with already implemented opioid program (Jobs and Hope – West Virginia), funding for medical equipment to assist with overdoses i.e., nalmefene HCl, naloxone, and fentanyl strip tests. I do not see having many challenges in getting partnerships with the AA programs and Jobs and Hope – West Virginia. However, I foresee challenges in accessing the medical equipment the program needs regularly for the number of individuals enrolled in the program. My fear is pharmacies may not be able to provide the medication on a basis that works for the individuals in the program.
