Concept explainers
To determine:
The nursing staff on Rick’s surgical unit have been reviewing their patients’ medical records and see a steady increase in the incidence of catheter-acquired urinary tract infections in patients over the last 3 months. The staff agrees to look into the current practice guidelines about the prevention of CAUTIs and determine if a change in practice is indicated. The appropriate PICOT question for this group and identify each part of the PICOT question.
Concept introduction:
PICOT is an evidence-based practice question which has five elements that give a clear evidence of scientific articles. This PICOT is a question which is focused on the nursing research. The well-organized question should not contain all of the following sequences of PICOT, but it has all the evidence of clinical research.
Explanation of Solution
PICOT question is the initial step in the evidence-based practice. P refers to a patient population of interest, C refers comparison of interest, O refers outcome of the intervention, and T refers to time. Some questions are evaluated without answering I, C and T elements. Many of the PICOT questions has only the P and O, but good research studies have to find out all the possible answer from this PICOT question. Nurse investigate the clinical problem and then create a PICOT question.
In this clinical application, P refers to Rick, patient. O refers to the incidence of catheter- acquired urinary tract infections. I refer the current practices which prevent the catheter-acquired urinary tract infection. C refers the comparison of the incidence of urinary infection over the last 3 months without treatment and after treatment. T refers the current time during the prevention of urinary tract infection.
Thus a PICOT question develops to all clinical application, but it is not necessary to answer all elements in a PICOT question. The PICOT question helps to collect them all possible information about the clinical case study by asking relevant questions.

He smoked two packs of cigarettes per day for more than 50 years and states he quit smoking 10 years ago. He also has a history of hyperlipidemia. 0800 The 77-year-old male client has a productive cough with thick, green secretions. Heart sounds are muffled. Breath sounds are diminished bilaterally with inspiratory and expiratory wheezes noted. The client denies pain, rating his pain a 0 on a 0-to-10 pain scale. He reports dyspnea (difficulty breathing). His chest is barrel shaped. The client ate only 25% of his breakfast today. Affective The patient will communicate feelings and worries about stigma in a safe atmosphere. Regardless of cultural differences, the patient will be more motivated to adhere to the treatment plan.
