She is also  incontinent of urine and stool and is confined to her bed or wheelchair. She recently had a  bout of pneumonia and was admitted to acute care. She has now returned to the nursing  home, having lost 7 lbs over the last three weeks. Include a brief reason for each factor.  What factors indicate that Mrs. Harris is at risk for the development of contractures? This is after Mr. Garcia been seen by a RDN and made changes do his diet and nutrition education before leaving the hospital. The changes included: 1. No added salt / low saturated fat 2. Low cholesterol 3. Pre-diabetic (based of glucose lab) 4. Has no food allergies 5. His wife does the majority of cooking and shopping (she started to use corn oil to cook with and not fry foods as often) 6. Household 5 (his daughter and two grandkids live with him and Mrs. Garcia) I suggested the following: 1. Mr. Garcia should consume more fiber (soluble and insoluble) 2. Mr. Garcia was interested in taking fish oil supplements (I was thinking added more omega-3 to his diet) 3. Mr. Garcia is very dehydrated (nursing assessment: skin turgor = TENT) So now that Mr. Garcia left and received education and made diet changes, we are now seeing him and his wife 4 weeks later as an outpatient.
Rashidah is a 55-year-old female who is experiencing acute chest pain and shortness of breath. She describes the pain as “pressure and squeezing.” When she becomes nauseous and light-headed, her son insists that he take her to the emergency room.   Health History: She started smoking when she was 18, attempted to quit several times in her 40s, and had her last cigarette on her 50th birthday. She has been overweight for most of her adult life but has gained 20 pounds in the last few years, which increased her BMI to 34. The last time she had a physical exam was when she turned 50. Her vital signs and blood test results from that exam are listed below.   BP: 178/90 HDL: 62 mg/dl LDL: 190 mg/dl Triglycerides: 174 mg/dl Total cholesterol: 252 mg/dl Fasting blood glucose: 128 mg/dl Rashidah quit smoking 5 years ago but still suffered a myocardial infarction. She is tempted to start smoking again since quitting “obviously didn’t make me healthier.” What does research tell us about her future…
