Chapter 14, Problem 1CCS
A Deadly Carrier
In 1937, a man employed to lay water pipes was found to be the source of a severe epidemic of typhoid fever. The man, who had recovered from typhoid, was still an asymptomatic carrier of Salmonella enterica serotype Typhi, the bacterium that causes the disease. He habitually urinated at his job site and in the process contaminated the town’s water supply with bacteria from his bladder. Over 300 cases of typhoid fever developed, and 43 people died before the man was identified as the carrier.
- 1. How do you think health officials were able to identify the source of this typhoid epidemic?
- 2. Given that antibiotics were not generally available in 1937, how could health officials end the epidemic, short of removing the man from the job site?

An outbreak of viral meningitis in a small eastern city was linked epidemiologically to a group who swam in a nonchlorinated pool in an abandoned quarry outside of town. What might public health officials surmise about the probable cause of the outbreak?
This 35-year-old woman reported that she had experienced three bouts of urticaria of unknown origin about 10 years ago. The urticaria affected her mucous membranes and skin. She had experienced similar symptoms after repair of a fractured femur caused by a skiing accident. These symptoms were attributed to an antibiotic reaction.
As an emergency room nurse, she observed occasional localized hives after the use of latex gloves. Even when she used hypoallergenic latex gloves, she continued to have hives every few months. Increased urticaria, at times generalized, continued to occur.
Within 30 minutes of having a routine vaginal examination performed by a health care provider wearing latex gloves, she had an anaphylactic reaction that required resuscitation and hospitalization. A vaginal biopsy 1 week later required a latex-free environment for her safety.
A short time later, she was forced to retire from nursing because of symptoms of asthma. She had also developed food allergies to…

- A 9 year old boy with cystic fibrosis – a genetic disease that causes a number of problems, including the build-up of thick sticky mucus in the lungs- complained of increasing fatigue, shortness of breath and worsening cough. When his mother took him to the doctor, she mentioned that his cough was a blue green color. His doctor immediately suspected a lung infection by Pseudomonas aeruginosa a common complication of cystic fibrosis. A sputum was collected and sent to the clinical laboratory. In the Clinical laboratory, the sample was plated onto Mac Conkey agar and blood gar and incubated. Mucoid colonies surrounded by bluish green color grew on both types of agar media. The colonies on Mac Conkey had no pink coloration, so the medical technologist concluded that the cells did not ferment lactose. She noted that the blue green color on the agar plates and in the sputum, knowing that P.aeruginosa makes several pigmented compounds that give rise to colors ranging from yellow to blue. The mother, at the time of admission, had complained of mild diarrhea and abnormal abdominal pain unrelated to her pregnancy. The infectious disease doctor who was called in to consult on the case immediately recognized the likely problem and ordered blood cultures be performed on the infant. The infant was also started on intravenous antibiotics. Two days later, the lab reported finding a Gram-positive bacillus-Listeria monocytogenes-in the infant's blood. This same organism was the cause of the mother's diarrhea. The mother had unwittingly ingested some unpasteurized cheese contaminated with this pathogen and developed listeriosis. The organism entered the…arrow_forwardWhy would medication fail to cure HSV infections even though it prevents recurrent cold sores?arrow_forward
