Your 21 yof patient fell out of a pickup truck traveling at about 30 mph.  She hit her head, was initially unconscious, and then became alert but confused.   Upon examination: LOC – responsive but confused Airway – patent Breathing – slightly slow with adequate chest rise SpO2 – 90% on ambient air Circulation – slow, strong, irregular pulse; pink, warm, dry skin; and no bleeding found Signs of injury found during secondary assessment: lowering level of consciousness, pupillary dilation; vomiting without nausea.   Vital signs: BP – 146/92; pulse – 52, strong, irregular; and respirations – slow, irregular   Treatment?   Rapid transport to level 1 trauma center.  Reassessment performed every 5 minutes.
