Sharon Micheals, a 23 y.o college student, has presented to the campus clinic with complaints of suicidal ideations. She has been admitted to the inpatient emergency facility on a legal hold. Her orders are as follows.
Admit for suicidal ideation
Suicide precautions
Regular diet
Routine VS
Buspirone 20 mg PO every 8 hours
Sertraline 50 mg PO BID
Haldol 2 mg IM q 1 hr PRN agitation
Lithium 600 mg PO TID PRN manic episodes
Lithium 600 mg PO q 6 hrs
The patient denies any medical history. Denies any current pregnancy and reports an LMP of 3 weeks ago and that she is taking a daily oral contraceptive. She states that she was diagnosed with depression at age 16 after she was in a car accident that killed her best friend. She was the driver of the car. She also reports that she was diagnosed with Bipolar disorder at age 20. Pt reports that she has felt isolated since fall semester has started and she feels overwhelmed by her workload. She went to clinic to get help with feelings and perhaps a referral to a counselor. She takes buspirone 15 mg PO as needed for feelings of anxiety, stated last does was this morning. She was prescribed an antidepressant but did not like the side effects and stopped taking it over the summer.
