Overview The B vitamins are found widely in animal and vegetable foods. The main functions of these vitamins are as co-factors for metabolic processes or as precursors of essential metabolites. Deficiencies in B vitamins can cause severe disease and/or death.   History of Present Illness JH is a 55-year-old homeless male patient presented to the emergency department with a 1-week history of progressive dyspnea and generalized edema. He had experienced several loose stools with intermittent abdominal pain localized to the right upper quadrant for four months. He denied smoking or using illicit drug use, but confessed excessive and protracted alcohol consumption and a dietary pattern restricted to intake of only carbohydrates; specifically, eating cookies and highly processed foods that had been thrown out by a local convenience store.  Diagnosis and treatment The patient was diagnosed with cardiac beriberi by the attending physician. Thiamine (vitamin B1) was administered intravenously (100 mg) followed by a daily intravenous infusion of 100 mg for the remaining hospital stay. With thiamine supplementation, JH's condition steadily and gradually improved. JH was given permission to leave hospital two weeks after admission.   Clinical Questions 1 What is beriberi? mild hyperventilation. He had icteric sclera without
evidence of anemia on physical examination. Jugular
veins were distended to the angle of the mandible.
Cardiac examination showed a regular rate with no
murmurs. Bibasilar crackles were auscultated in both
lungs. There was a tender, enlarged, and palpable liver
below the right costal margin. Both lower extremities
were warm to touch and showed edema in a
symmetrical fashion. Neurologic examination revealed
no abnormalities.
