Scenario
Hill View is a fictional residential care home; home to residents with a
wide range of care needs including Dementia. Over the past few months,
a number of residents at Hill View Nursing home have required hospital
treatment and care for conditions the GP felt should have been effectively
managed by the nursing staff at the home.
The GP feels that some of the residents have experienced; poor
management of diabetes care which resulted in two patients becoming
critically ill. One gentleman resident developed sepsis possibly due to
poor care of pressure sores and another lady's health significantly
deteriorated following the home not ordering her repeat prescription and
her not receiving her medicines for a five-day period prior to hospital
admission. As there are a number of cases, the GP has Safeguarding
concerns.

Another resident stated that staff changed all the time which they found
stressful and upsetting because their care, particularly personal care
needs was being 'handled by strangers' who didn't know how they liked
things done.
One patient with Dementia seemed to be confined to her room despite her
seeming distressed by this. When the GP asked he was told by a care
assistant it was for her own safety.
