Your job is to create a summative assessment for the following program and objectives.

 

You work for the NYC Department of Health and Mental Hygiene.  You will be working on a 5-lesson presentation to 8th grade students during their health class in the middle school.  You teach one lesson per week for 45 minutes per lesson.  This is a program to help students recognize the warning signs of cardiovascular and the risk factors of cardiovascular disease.

The program objectives are below.

  1. Students will be able to list 5 warning signs of a heart attack after participating in a workshop and reading a pamphlet on heart attack from the American Heart Association.
  2. Students will be able to Identify 4 risk factors for developing heart disease after reading a pamphlet on heart disease from the American Heart Association.
