Mr Anthony Drinkwater was a 62 year old gentleman who was brought up in an orphanage. For many years he was chauffeur and gamekeeper for Sir Cyril Blanchmore, a Baronet who owned an estate in Stirlingshire. Alas, the estate had run into financial difficulties and Mr Drinkwater was made redundant. Mr Drinkwater drifted south to London. He took casual work in restaurant kitchens and frequented pubs in the city. He lived in a bed-sit and gradually slipped downhill, drinking cheap sherry and cider. Eventually he was found unconscious in a public park during a very cold spell. He was admitted to the geriatric ward of the Hospital. He had peripheral frost bite of his toes and grossly oedematous legs. He smelled of acetone and was found to be dehydrated. His chest was consolidated with little breath sounds. A throat swab and also a blood specimen for culture both were sent to the lab. The blood had a high number of white cells and these were mainly neutrophils.     what's the diagnosis? thank you!
