A heavy load of lumber badly hurt a lift truck operator while she attempted to dislodge an obstruction from the underside of a pallet. An investigation determined that the operator apparently climbed between the mast and underside of the pallet to remove an obstruction (there were no witnesses to the incident). Either she slipped, or the load shifted, catching her under the pallet that was stacked with lumber. The operator had been doing the job for 15 years, with over 10 years behind the wheel of a forklift. Her training was current and she had an excellent safety record.
