A volcano is a conical or domed-shaped hill formed by the extrusion of lava or any pyroclastic material. A volcano is a landform that formed due to volcanism. 

A volcano is a fracture or rupture in the Earth's crust from which volcanic gases, lava and ash erupt. 

Magma is less dense than the overlaying rocks so it's always trying to move up. It always comes through some weak zones like fractures. 

The process of eruption of molten material into the Earth's crust is known as volcanism.
