Identify three agencies in your local community that support individuals in later adulthood. Living in a rural area in Alabama, it may be difficult to locate community agencies for individuals in later adulthood. The agencies that are available in our area are a great resource for the elderly population. One agency that is available is the Alabama Tombigbee Area Agency on Aging (ATRC). This agency is part of a state-ageing network that services several counties including Clarke County which is where I reside. ATRC offers a variety if services for seniors, disabled and caregivers. This agency works with other public agencies and businesses to provide a range of healthcare needs and social services to aid in maintaining independence and/or improving the quality of life for the aging population. ATRC provides programs founded by the Federal Older Americans Act as well as programs that are funded by the state of Alabama. The next agency available is that of a long-term care facility, Jackson Healthcare. This facility is a skilled nursing facility which offers both short term and long-term placement. The long-term placement is for the elderly that are no longer able to be cared for in the home whether it be due to medical conditions or limited caregiver support. The third agency is the Department of Human Resources. The Department of Human Resources (DHR). Provides services through adult protective services. Adult protective services is to support and enable County Departments to protect elderly and disabled adults from abuse, neglect, and exploitation and prevent unnecessary institutionalization.
Provide an overview of the services each provides. Alabama Tombigbee Area Agency on Aging (ATRC). ATRC provides many services to older adults. The most used service is that of Meals on Wheels. The Meals on Wheels program is designed for a homebound person to receive a meal delivered to their home Monday-Friday. To participate in the program, you must be 60 years of age and homebound. There are also nutrition centers available for those that can come in. The nutrition centers provide not only food but socialization, recreation, and education to those who participate. The Elderly and Disabled (E&D) Wavier is also a part of the ATRC program. The Medicaid Elderly and Disable Wavier Program provides services for seniors and the disabled whose needs would otherwise require nursing home placement (ARTC). This program provides case management to elderly people and their caregivers. The case manager establishes a plan of care that may include services such as homemaker, personal care, respite, adult day health and meals delivered to the home. The goal of the E&D Waiver program is for the client to be able to maintain their health and care while remaining in their own home as long as possible. Jackson Healthcare Facility This is a 75-bed licensed skilled nursing facility. The facility accepts Medicare, Medicaid, HMOs, private pay and supplemental or long-term care insurance which will cover the cost. The facility has licensed and certified medical staff as well as physician coverage for the residents. The facility offers a friendly and homelike

environment to its residents and visitors. Many long-term care facilities are needed; however, many people prefer to keep their loved ones at home. Placement at Jackson Healthcare is often necessary when the medical needs of their loved one are no longer manageable in the home. Long term placement is also needed when there is not a caregiver present. Long term care facilities are inspected yearly and as warranted by the state of Alabama. Department of Human Resources The Department of Human Resources (DHR) follows the APS Act of 1976 which addresses the abuse, neglect and exploitation of adults who are not capable of protecting themselves or may not have a person that is willing to protect them. It is under this act that the DHR is to receive and investigate the reports of suspected abuse, neglect, or exploitation of a person. Under the APS Act of 1976 DHR can arrange any necessary services that will be put in place to protect a vulnerable adult. Identify the service gaps you notice. There are often gaps in services because the older adults may not have the social support available to them. Protective factors rest on a stable social network, meeting social needs, and the increasing use of social technology (Teater,B 2021). The elderly may not know what resources are available to them in addition to the coverage that they already have. Not knowing that they can have additional benefits causes them to lose out on extra coverage. Explain how to improve existing services to address these gaps.
There can be community health fairs or educational events to provide information to the elderly population.
