What physical changes occur related to the onset of the disease? What are the possible psychological changes with PD? What system(s) are affected? What is the prognosis? What are the treatments and therapies? Parkinsons disease is a progressive disorder that affects the nervous system and the parts of the body controlled by the nerves. At the onset of disease, a patient may notice a tremor in their hand or stiffness. It is estimated that 60,000 new cases of Parkinson’s disease are diagnosed each year, adding to the estimated one to 1.5 million Americans who currently have the disease. There were nearly 18,000 Parkinson’s disease-related deaths in the United States in 2003. While the condition usually develops after the age of SOC313 1
55, the disease may affect people in their 30s and 40s, such as actor Michael J. Fox, who was diagnosed at age 30 (Parkinson’s Disease – Symptoms, Diagnosis and Treatment, n.d.) The potential psychological changes in PD patients seem to be exhibiting psychological symptoms, including despair, anxiety, hallucinations, misperception, apathy, and Snowdonia, as well as obsessive and impulsive behavior and cognitive problems. The nerves govern both the neurological system and the body's components. Parkinson's illness worsens over time if left untreated. Parkinson's disease has the potential to cause premature mortality and a decline in all brain functioning. Parkinson's disease has no known cure. However, medications, surgery, and other therapy can frequently lessen some of the symptoms. The case study shared details that indicate Doug is no longer able to identify and resolve problems. Identify two examples in the study that indicate Doug is losing this ability. Provide two other examples that may arise as the disease progresses that are related to identifying and resolving problems. Doug was exhibiting stopped posture and changes in his speech which made it difficult to understand what he was saying. After he lost the ability to stand up, even with help, his condition progressed to the point of needing full-time care in a skilled nursing facility. He also couldn’t control his own medications. According to (Cognitive Changes, n.d.) Some people with Parkinson’s disease (PD) experience mild cognitive impairment. Feelings of distraction or disorganization can accompany cognitive impairment, along with finding it difficult to plan and accomplish SOC313 2
tasks. Two other problems that may arise could be difficulty with keeping up paying your bills or not being able to complete a recipe. Discuss possible feelings and stressors Doug may be experiencing as the disease progresses and he must rely on caregivers more. Refer to the required reading list for the topics related to this assignment for ideas. He started feeling like he did not belong around his friends and family. He began to struggle with doing activities of daily living and it was hard for him to play scrabble. His speech started to get delayed which made it hard for people to understand what he was saying. It became so bad that he did not want to participate in weekly game nights. Dough probably felt scared also because he knew he was losing parts of himself and worried because he doesn’t know what is going to happen to him. Discuss the effects the diagnosis of PD and its progression can have on a spouse and family (micro-level). Refer to the chapter and required readings regarding caregiving and caregiver burden (e.g., employment, psychological effects, financial stressors, role in the marriage, etc). PD can affect family members just as much as the patient. Parkinsons can affect mood, self-image, and communication. This can cause arguments in a marriage or where the spouse can’t understand what the patient is needing or communicating for. The patients themselves may feel guilty because they can’t help in the household. The spouse may feel resentment because they are taking care of something 24/7. The family can be affected because it can change how they respond to things. Vacations and family events may have to be cancelled. Having to pay for a treatment may place a financial burden on the family and spouse. SOC313 3

Locate two services in your own community that offer assistance to people with PD and their caregivers (meso-level). Describe the services available, cost, and eligibility requirements. In your opinion, are Doug and Maria eligible for the services? Explain your answer based on your findings and knowledge of Doug and Maria. Mid-South Parkinson’s Disease Support It provides education and support to Parkingson patients, their families, and their caregivers. The organization consists of volunteers. The group gives caregiver tips, classes, and helps a patient or family member find support in their general area. It’s free because it’s volunteers. They even have free exercise classes twice a week that is geared for Parkinsons patients and caregivers can join in. 901 Parkinsons Fighter This is another non-profit organization that provides programs and resources to people with Parkinsons, their families, caregivers, and the community. I believe Doug and Maria are eligible for services because it’s a not-for-profit group. Both companies help the patient and caregivers find help through their networks and programs. The exercise classes are free. I believe they may be eligible for services that the group would help them find because he has Medicaid. Most states offer community help through the state where they can get a caregiver to drop in. At the macro-level, research the state and federal programs, legislation and initiatives that affect chronic illnesses. Discuss the pros and cons of two of the following items as they affect “Social Security benefits provide a critical safety net for millions diagnosed with Parkinson's. Social Security Disability Income (SSDI) requires information about your medical condition, work, and education history to help it decide if you are "disabled." One SOC313 4
patients and caregivers. o Requirements for Social Security Disability Income [SSDI], o Requirements to take advantage of the Family and Medical Leave Act [FMLA], o benefit is that SSDI pays benefits to both patient and a family member if they worked long enough and paid Social Security taxes ( Insurance , n.d.).” Once con is that there is a five- month waiting period to receive disability payments once approved. “The Family and Medical Leave Act (FMLA) provides certain employees with up to 12 weeks of unpaid, job-protected leave per year. It also requires that their group health benefits be maintained during the leave. To be eligible to take FMLA, you must have been at employer for at least 12 months, 1,250 hours, and employes 50 or more employees within 75 miles ( Family and Medical Leave (FMLA) , n.d.)”. The pros are that if you need to take leave, FMLA will protect your job while on leave. Another pro is that they will keep your health insurance during that time. The con is that to be able to take it; you must have been employed for a minimum of 12 months for it to take effect. Another con is that one only gets paid while on leave. Not receiving a paycheck would be complicated with someone who has a disability because they or their family member may still need to work. If the family member needs to take FMLA to care for their loved one at any time, they wouldn’t get paid, and they might rely on that job for income.
