Disability Report & Intervention You are going to interview a person with a disability or illness. This can be any disability or illness. During this interview, you are going to ask about the client’s demographic: 1. Name (fake name) 2. Age 3. Ethnicity, cultural background 4. Occupation/previous occupation 5. Disability/illness a. Symptoms or experiences the person encounters 6. Circle of support (formal and informal supports) In addition, you will use the following two assessments to understand the client’s needs and interests: leisure interest measure and leisure motivation scale. You will only use the assessment to understand more about the client, you will not be scoring. After this assessment of the client, you will create an intervention for this client that will provide support to their disability/illness while keeping in mind their needs and interests. The intervention will include an activity analysis/program plan. Discussing goals, objectives, frequency, and all information to explain this intervention in depth. These 3 forms once completed will be placed in the appendix section of the paper. You will write in paragraph form to describe the client (including all important information). You will research more about the client’s disability/illness and include this supportive information when discussing the client’s disability/illness. You will describe the intervention in detail and research the intervention to provide evidence-based information about the intervention (i.e., benefits). Grammar/Structure a. APA Format b.
