D.Q. 8.1 discuss a case conceptualization for BPS factors and CRSJ considerations that contribute to anxiety and depression in adolescence. Using a case conceptualization for an adolescent with anxiety and depression, would help create a coherent treatment strategy, while focusing on treatment interventions that increase the likelihood of achieving the treatment, and when the therapist is confident in the plan and communicates this to the client it can build a lot of trust and belief, which can make the difference in therapy (Sperry & Sperry, 2020). An example of case conceptualization for this adolescent would be obtaining information about the presenting problem like when it started, if something triggered it, genetic predispositions, and other things in my client’s life whether it be current or past that may be contributing. My example client is a 16-year-old girl, who suffers from chronic pain due to arthritis in her hip and has to walk with a cane at school while she is waiting for surgery. This causes her to get a lot of unwanted stares or attention at school and in other social settings causing her to miss school more frequently and not go out with friends. She already feels out of place and that she doesn’t culturally fit in with her friends due to being biracial. Her family has a history of suffering from anxiety and anxious attachment styles due to the death of a parent. A strategy for treatment for this adolescent is helping them to develop coping strategies for dealing with stress, counsel her for the grief she may still be feeling from her parental loss and help her develop cognitive and behavioural strategies to overcome the social anxiety (Shebib, B., 2020). Based on the case conceptualization framework from Sperry & Sperry (2020), the case conceptualization for john is: Presentation: Unhappy in multiple areas of life including his relationship, avoiding work, fatigue Precipitant: Currently feeling controlled by his partner leading to his dissatisfaction, has felt inadequate to other for a long time due to his parents comparing him to his siblings and putting him down. Pattern maladaptive: Sleeping all day while avoiding work Predisposition: Biological: potential family history of alcoholism and poor mental health. Psychological: feels sad, has issues with feeling controlled. Current social: feeling alienated by co-workers, in the past his parents always compared him to his siblings and highlighted his shortcomings.
Perpetuants: Lack of support from His romantic partner, his family, his partners family and at work. Also, has irregular sleeping patterns. Protective factors: Not certain, relationship with his children and his siblings should be explored further. Cultural identity: Not overly clear, presumed to be a minority but identifies as a Canada, bisexual. Cultural stress: deals with high acculturative stress due to possibly being from a different culture than his partner his in laws disapproval of the relationship, along with prejudice comments made by his co-workers. Cultural explanation: thinks that his challenges and issues in his relationship are from “choosing” the wrong sex to be with and that his partner is “controlling and overbearing”. Culture an/or personality: Both culture and personality dynamics are operative. I find it hard to address cultural aspects in regards to john specifically because although he identifies as Canadian, Canada is often referred to as a mosaic because many different ethnicities, races and cultures contribute to Canada being what it is today.
