What are at least two consequences of misdiagnosing bipolar disorder as unipolar depression? Bipolar disorder is a complex disorder on its own and requires an appropriate diagnosis. When patients are misdiagnosed, it can lead to treatment complications, delay in treatment, as well as recurrence of symptoms. Often, patients with bipolar disorder may be diagnosed with unipolar depression. If this happens and the patient is started on an antidepressant monotherapy, the patient with bipolar disorder can experience a manic episode. In addition, the treatment can also trigger rapid mood cycling, which can lead to unnecessary symptom burdens for the patient and family. Patients with bipolar disorder also need to be on mood stabilizers. When misdiagnosed, the treatment with a mood stabilizer may not happen. As a result, the patient may attempt suicide or need to be admitted to the hospital, which would all lead to increased healthcare costs and negatively affect the safety and well-being of the patient. In addition to high health care costs, the patient would also lose days from work and incur productivity loss. Unipolar disorder can be treated with antidepressants, but bipolar disorder cannot be treated with antidepressants only, as it could trigger a manic episode. As explained previously, monotherapy with antidepressant can lead to mania and trigger rapid mood cycling. Patients with bipolar disorder respond well to lithium treatment, whereas lithium may only be used adjunctively in some cases of unipolar disorder. Lithium is considered first-line treatment for maintenance therapy for bipolar disorder. Patients with bipolar disorder can be treated with atypical antipsychotics such as quetiapine.
