I appreciate how you mentioned documentation. Documentation, in my opinion, is an ethical dilemma within itself. For example, if I had a patient/client who exhibited behaviors that put others at risk of harm, documenting that incident would be critical to demonstrating that I was able to diffuse the situation and help maintain a safe environment for that patient/client and others within the environment. However, another side to this accurate documentation would be that if that patient/client needed additional
services or a change in environment, other entities may decline to accept that patient/client based on their history. With records request, clients and sometimes their families are able to access the records. It is important to ensure dignity and respect for the client through our documentation (NASW, 2021). It is also important to challenge social injustice of the bias and prejudices that inhibit appropriate services for clients (NASW, 2021). I enjoyed your post, thank you for the enlightening material.
