Case Study for group work Name: Elizabeth McLean – known as Betty Age: 79 Medical History: Mild Cognitive Impairment Hypertension Migraines Urinary Frequency Current Prescribed Medication: Ibuprofen 400mg, up to 3 x daily Amlodipine 5mg, 1 x daily Oxybutynin 5mg, 2 x daily Height: 1.58m Weight: 47kg BMI: 19 You are the assigned Community Mental Health Nurse to Betty, who you provide support for following her recent diagnosis of Mild Cognitive Impairment. When you begin work on Monday morning, you look up Betty’s file and discover that she has been admitted to the Geriatric Assessment Unit at Aberdeen Royal Infirmary following a fall at home on Friday evening. She has sustained a distal radius fracture. You are surprised that on Betty’s admission document, the admitting physician has written ‘severe dementia’ and ‘vivid, distressing hallucinations’. You note that Betty now has Paracetamol 1g, up to 4 x daily has been added to her medication regime. You call the ward and speak to the nurse assigned to Betty for the day, given your concerns about the notes made in her admission document. The nurse informs you that Betty has had to be allocated to a room on her own as she was awake through the night getting up from her bed and screaming at other patients and staff. The nurse tells you that Betty is extremely tired, confused and disorientated and she is becoming ‘very aggressive’ when staff try to offer assistance with personal care and nutritional support. The nurse notes that Betty is currently dehydrated and that she is very hard of hearing. You inform her that Betty usually wears glasses and hearing aids, both of which she does not have on the ward currently. You discuss that Betty’s current presentation is very out of the ordinary and that she appears to be far from her baseline.
