Reflection on Practicum Experience Degree of Achievement of Objectives Throughout my clinical practicum, I successfully achieved multiple objectives in the field of psychiatry. Actively participating in psychiatric case conferences, grand rounds, and diagnostic exercises, I honed my skills in developing accurate differential diagnoses. Thorough study of the DSM-5-TR and engagement in case discussions, alongside collaboration with my preceptor, significantly contributed to my ability to formulate precise psychiatric diagnoses. The development of comprehensive psychotherapeutic treatment plans was achieved through studying evidence-based interventions, attending therapy sessions, and collaborating with my preceptor to tailor plans to individual patient needs. Additionally, I learned to interpret psychiatric screening instrument results through hands-on experience, enabling me to develop personalized care plans based on these results. Seeking guidance and feedback from my preceptor played a crucial role in enhancing my proficiency across these objectives. Challenges Encountered During my clinical practicum, I faced several challenging patient encounters, with three instances standing out as particularly demanding. One involved a patient diagnosed with borderline personality disorder, presenting difficulties due to symptom overlap with various psychiatric conditions. Another challenging case was a patient with puerperal psychosis, a medical emergency necessitating consultation with a psychiatrist and subsequent referral to the primary care physician for comprehensive obstetric management. The third challenging encounter involved a patient with schizophrenia, exhibiting delusional and aggressive behavior, accompanied by persecutory hallucinations. In response to the perceived threat, we had to
administer diazepam for sedation, which unfortunately led to respiratory depression, escalating the situation into a medical emergency. Urgent intervention involved the intravenous administration of flumazenil as the patient was swiftly transferred to the emergency department in the primary care facility. During my practicum, lessons from overcoming challenges underscored the importance of collaborating with a multidisciplinary team and consulting research literature (Åhlin et al., 2023). Evidence-based practices, including utilizing diagnostic criteria like DSM-5-TR and tailoring treatment plans based on guidelines, ensured a comprehensive care strategy (Åhlin et al., 2023). To enhance skills, proactive engagement in diagnostic workshops, additional training on challenging cases, and building a more extensive community network for specific patient needs are identified areas for improvement (Åhlin et al., 2023). Managing Patient Flow and Volume and Social Change Agent Role Effectively managing patient flow involves efficient collaboration with the healthcare team, clear communication, and prioritizing patient needs. My growing skill set allows me to contribute positively to patient care and streamline processes. Time management and coordination with colleagues play a vital role in optimizing patient flow. Applying my growing skill set to be a social change agent involves community outreach, education, and advocating for mental health awareness. I aim to contribute to reducing stigma, improving access to mental health services, and promoting evidence-based practices within the community. Continued collaboration with local organizations and active participation in mental health initiatives will be key to fulfilling this role. Communicating Feedback

I plan to elevate my skills and knowledge by consistently seeking feedback from my Preceptor and actively engaging in self-assessment. Creating an environment of open communication is a priority, with scheduled meetings dedicated to discussing my progress and identifying areas for improvement. I want to foster a collaborative learning atmosphere where constructive feedback is not only welcomed but also valued for its role in personal and professional development. During these feedback sessions, I intend to directly address questions about my performance, seeking specific insights into both my strengths and the areas that require attention. By combining formal feedback with regular self-reflection, I aim to gain a comprehensive understanding of my progress and identify specific areas for growth. Consistently seeking feedback and engaging in self-assessment, especially from a Preceptor or mentor, is essential for personal and professional development, offering specific guidance for skill enhancement.
