2 Care Coordination Presentation to Colleagues Healthcare provision deals with many issues, and caregivers need to understand that care coordination and patient safety should be adhered to at all times. This can further lead to improved adherence to workplace ethics that promotes cultural and psychological aspects of the patients. Thereby, caregivers have to follow various guidelines about handling patients and providing care to improve the quality of care and ensure that the trust of the patients in the system is upheld by satisfying their needs. Furthermore, care coordination ensures that patients and caregivers connect personally and that all the arising issues are resolved while ensuring that each party is satisfied with the resolutions and care progress. It should be noted that care coordination makes the caregivers understand the patient’s practices while the patient gets more informed about their condition. Hence, the presentation will cover the aspect of care coordination in health facilities and the strategies that can be used. Effective Strategies for Collaborating with Patients and Families Collaboration in health facilities is vital as it improves the general outcomes of the patients and the facility. This needs the caregivers to collaborate well with the patients and families to achieve the desired outcomes. One way the caregivers can collaborate with the patients and families is through patient education. According to Morley and Cashell (2017), patient education is essential in ensuring that caregivers and patients understand each other on measures that need to be implemented. If a patient is being discharged, they need to be educated about how they can manage their health at home to recover well. Thereby, a nurse educating the patient means that they are empowering the patients and their families about managing the prevailing condition without causing complications. The article notes that patient education provides the patient and families with knowledge about medication use, making it easier to
3 prevent medication errors in a home setting. The approach further ensures that the patients and families improve their knowledge about issues and make it easier to apply preventative care. Another strategy that the caregivers can utilize is communication between the families and patients. According to Granheim et al. (2018), effective communication is key to achieving quality care and improved collaboration as it ensures that both parties voice their viewpoint and thus agree on the areas that need to be implemented. It is vital to point out that this means a patient will receive the care they deserve as they communicate their preferences which help the nurse make better clinical decisions on the treatment plan. Also, communication helps patients get urgent help if their condition gets worse, and the caregivers can consult with others to help the patient regain their health. Lastly, communication ensures that the patients and families can get clarification on guidelines that they are to implement at home and prevent the progression of the disease. Aspects of Change Management In introducing new practices or interventions, change management ensures that they are accepted and easily implemented. Hence, change management lays out steps that one can follow to implement new changes or practices. According to Harrison et al. (2021), stakeholder involvement is a critical element that directly affects patient satisfaction and care quality because it involves incorporating actions that affect people. This means that if patients are not involved in implementing certain practices, they might receive poor care due to the practices infringing on their rights or beliefs. This is also the same on the side of the nurses as they cannot properly an intervention they were not involved in, which means that the quality of care and patient-centered care can be compromised due to lack of enough information. This further means that the patients cannot receive value-based care and thus causing increased readmission to the facility, indicating

4 a fall in the quality of care. Also, communication is another aspect that is critical to patient satisfaction as it ensures that they have relevant information about their condition. Harrison et al. (2021) point out that communication makes the patient aware of the changes in medical procedures and how they can affect their health or experience. Thus, communication gives the patients full confidence in the care process, and caregivers have enough time to implement the new practices without clashing with the patient. This further ensures that the caregivers can sustain the new processes and continue applying evidence-based practices to improve care in health facilities. Rationale for Coordinated Care Plans Healthcare decisions have to adhere to various principles so that the patients' rights and ethics are not compromised. This is because there are several consequences that nurses can experience if they fail to adhere to the laid down procedures. Hence, it is always advised that caregivers have to use the available information to make decisions and overcome any dilemma and meet the needs of the patients. According to Varkey (2021), ethical decision-making is essential as it helps caregivers avoid conflict with the patients, which can derail care and lead to poor outcomes. This means that the patients’ views are listened to, and they are provided with reasons why certain procedures need to be done and ways the patients’ beliefs will be protected. The approach makes the patients cooperate well and even allows the application of interprofessional collaboration to manage their condition. By adhering to the principles of ethical decision-making, the caregivers are shielded from violating the rights of the patients or their culture, which can cause a breach of trust. Another important reason why nurses have to uphold the practice is that the patient can receive relevant interventions that do not jeopardize their health through it. Hence, nurses have to practice high standards of care and ethical decision-
5 making to avoid legal procedures and disciplinary processes from various healthcare agencies. Impact of Healthcare Policy on Outcomes and Patient Experiences Healthcare policies have been vital in improving care provision in communities and enabling members to access care easily. According to Silberman (2020), the Affordable Care Act has enabled communities and vulnerable populations to access affordable and high-quality care since its inception. Through the policy, the health insurance providers were directed to cover all conditions and should not exclude pre-existing diseases like it was in previous cases. This condition made it possible for the children to be covered under their parent’s policy without extra charges. Its impacts have led to the enrollment of over 76million for Medicaid and 64million for Medicare in the American population leading to early diagnosis of diseases and provision of urgent care. Th approach has further meant that patients can get medications that they could not afford previously, making them achieve better outcomes and reduce health disparities. Also, the policy’s expansion has led to an increase in community health centers that ensure that community members access preventative and urgent care. It should be noted that the policy has laid out procedures that caregivers have to follow to improve the quality of care. Conclusion In conclusion, care provision is a practice that greatly relies on the need to collaborate between the patients, families, and caregivers to achieve the best outcomes. Thus, the nurses need to understand that applying relevant strategies makes the patients satisfied with their quality of care and cooperate well. It should be noted that, even with increased collaboration, caregivers have the responsibility of identifying change management elements that can impact patient
6 experience and developing ways they can be mitigated to promote quality care and improved use of interventions. Lastly, ethical decision-making is important as it ensures that the rights and needs of the patients are respected, and health policies like ACA make it easier to achieve healthy outcomes in communities.
