Analyze the current state of TPMC and the projected market segmentation for TPMC anticipated from the proposed affiliation. Your analysis must be supported by demographic data and include the following elements:

• how the population’s health needs align with the proposed affiliation According to the latest analysis of TPMC, patients are excessively relying on the emergency room for diabetic treatment because they lack alternatives. Although TPMC is committed to serving the uninsured and underserved population, allowing these patients to access emergency care frequently creates a significant financial burden on the hospital and hinders
the staff's ability to provide quality care.
Segmentation in healthcare aims to classify the population based on their health status, healthcare needs, and priorities (Segmentation Approaches – Outcomes Based Healthcare, n.d.). In light of this, TPMC intends to transfer the group of patients whose healthcare needs they cannot cater to, to an organization of their choice that can address those needs. It's important to note that TPMC does not want to eliminate patient care altogether.
B. Affiliate Recommendation

Recommend an affiliate based on your goals and rationale from part A and include key attributes of the proposed organization that support the affiliation.
The recommendation I propose to partake in this venture with TPMC is The Mayo Clinic. The Mayo Clinic offers a thorough diabetic education program, as well as pediatric diabetic specialty care (Diabetes - Care at Mayo Clinic - Mayo Clinic, 2023).

Identify the major benefits of affiliation with this organization and describe how these benefits strategically align with TPMC’s goals.

Ochsner Health
Although Ochsner Health is a highly ranked hospital in Louisiana, with dedicated specialists and quality services (Ochsner Health, n.d.), it does not prioritize patient-
focused care. They offer financial assistance, but it only covers deductibles or
copays, leaving uninsured, underinsured, and underserved patients with a significant financial burden. This goes against the mission of TPMC, which is to provide care to
patients regardless of their ability to pay.
Franciscan Missionaries of Our Lady Health System
Franciscan Missionaries is a large healthcare system based in Louisiana and is
the leading healthcare provider for over half
of Louisiana's population. They claim to be dedicated to providing high-quality care for patients with diabetes across their health system (Diabetes Services | Endocrinology |
FMOLHS, n.d.). While they do offer financial assistance programs for the uninsured and underserved, the assistance is limited to medically necessary care in emergency situations only. Although diabetic emergency situations could be covered, the
aim of the affiliation is to partner with an organization that can provide care for patients even in non-emergency situations. Consequently, uninsured and underserved patients would likely be turned away or treated and then left with a hefty bill to pay.
This goes against everything TPMC stands for, and therefore, the affiliation would not work with Franciscan Missionaries.

C. Affiliation Plan
1.
Describe your proposed affiliation structure and include justification for the selection.
The affiliation structure will function as an integrated clinical network. This implies that patients can obtain care for their primary needs at TPMC primary care, receive specialty care at The Mayo Clinic, and all physicians involved in the care will remain informed of what the other organization is doing. They can discuss a plan together that best suits the
needs of the patient's healthcare.

Create new organizational, vision, mission, and value statements for the new proposed affiliation structure. Organizational Statement
I propose that TPMC's organizational statement be maintained as follows: Twin Pines Medical Center maintains organizational flexibility to allow for quick responses to medical service opportunities that improve and promote healthy lifestyles, regardless of race, social or economic status (Twin Pines Medical Center, n.d.).
Vision Statement
To become the leader in improving healthcare quality, at no cost for the underserved patients in our communities.
Mission Statement
To provide affordable, quality healthcare services to all communities, regardless of demography.
Value Statement
Our commitment to putting people first will ensure that everyone has access to affordable and quality medical services.

Develop a leadership plan for the proposed affiliation structure that includes the following elements:
strategies for overall organization management
strategies for the collaborative decision-making process
evidence-based principles, strategies, and tools of effective change management
The healthcare organization where a patient receives their care will be responsible for creating a leadership plan based on their specific needs. For instance, if a patient receives primary care at TPMC, they will decide the best course of action for the patient's care. Similarly, if they are receiving specialty care at The Mayo Clinic, The Mayo Clinic will be responsible for determining the best course of action for the patient's care. Both organizations will work together towards the common goal of providing top-notch care to patients, regardless of their paying ability, without causing any undue harm or stress to the patient, staff, or either organization. The management team of the organization will include the CEOs from The Mayo Clinic and TPMC, along with an integrated executive level board consisting of one COO, one CMO, one 4
CNO, one quality assurance officer, and one legal representative from each organization. This structure ensures that the affiliation is managed by both parties to prevent any bias towards one group over the other.
To ensure a successful affiliation between two organizations, it is crucial to maintain open, honest, and reliable communication channels. If any concerns or issues arise, the CEOs of both organizations should come together to peacefully resolve them and avoid terminating the affiliation. Any decisions made by the CEOs will be communicated to the CMOs and CNOs, who will then discuss the information with department leads. The leads will distribute the information to their teams. The same process will be followed in reverse, in case of issues, but the legal and quality assurance teams will also be involved.

Describe how to maintain and improve employee satisfaction, engagement, and retention in a culture of positive change. It's important to consider the possible tension that may arise when two organizations come together. This phenomenon is called "culture shock," which is the feeling of disorientation experienced by someone when they are suddenly exposed to an unfamiliar culture and way of life (Culture Shock Stages: Everything You Need to Know, n.d.). This could affect the staff of both organizations since they work for organizations with different missions and statements.
To reduce tension, it is important for staff to receive proper training and have the necessary tools to provide the best patient care. Staff who care for these patients, whether they are TPMC staff or The Mayo Clinic staff, are likely to feel intrinsic or extrinsic motivation. Intrinsic
motivation comes from within, when you are intrinsically motivated, you engage in an activity because you enjoy it and get personal satisfaction from doing it, while extrinsic motivation arises from external factors (Barr, 2022).
Throughout the implementation of any program, it is of utmost importance to ensure that the staff members involved are taken care of and their well-being is always prioritized. To achieve this, it is highly recommended to regularly check in with both groups of staff to inquire about their well-being and gather any feedback or suggestions on process improvements. It is imperative to involve staff in these discussions as excluding them from the decision-making process would hinder the program's progress and create an environment of mistrust and dissatisfaction among staff members. Therefore, it is essential to make staff members feel valued and appreciated by including them in the program's implementation process.

The staff at TPMC will be equipped with specialized knowledge and understanding of diabetic patients. TMPC may face budget challenges operating their medical center, which could hinder the addition of a specialty clinic to their hospital.
Although patients can still choose to use TPMC's services for urgent needs, they now have the option to seek care from another organization that they trust to take care of them.
Patients may be hesitant to seek medical care at a different facility due to concerns about transportation or discomfort with receiving care in an unfamiliar environment.

Discuss the impact the organizational cultures of TPMC and the affiliate organization could have on the future of the proposed affiliation.
The transition from TPMC to The Mayo Clinic is likely to bring about some notable changes in
the quality of care provided to patients. There may be a shift in the type of care offered, which could have a significant impact on patients who are used to receiving a particular type
of care at TPMC. It's important to note that the move to The Mayo Clinic also has the potential to bring about positive changes in the level of medical care and services available to patients. However, it's understandable that patients may feel uncertain and unsure about what to expect from the new facility. It's crucial that patients receive clear and concise information about the transition, the services available at The Mayo Clinic, and any changes in their medical care.
7.
Discuss reasons for termination of the affiliation agreement.
The reasons for terminating an agreement between TPMC and The Mayo Clinic could be due to various factors, but some significant ones include patient outcomes and performance metrics. If patients are dissatisfied with their care or if the care provided does not meet TPMC's standards, then the agreement may be terminated. Additionally, performance metrics play a significant role in potentially terminating the agreement. These metrics are used to measure the behavior, activities, and performance of a business (Sage, 2023). If The
Mayo Clinic fails to provide the required care, then the agreement may be terminated. Furthermore, if the ED at TPMC continues to receive an influx of diabetic patients, it renders the program pointless, and termination of the agreement would be warranted.
8.
Provide a potential exit strategy that is actionable and includes three action steps.
It is crucial to decide on an exit strategy before entering into an agreement. This way, if the need arises, both organizations will have a clear understanding of what is expected. The exit
strategy will consist of three steps: an initial meeting, the execution date, and the exit date. During the initial meeting, which can be called by either organization, the board of directors from both organizations will come together to discuss the issues and reasons for terminating
the agreement. They will then set an execution date, and patients will be informed of the anticipated termination, after which the exit strategy will go into effect. The final step will be the exit date, which marks the official termination of the agreement. On this date, all medical care will cease, staff will return to their respective organizations, and patients will be notified of the termination of the agreement.
9.
Discuss future challenges and opportunities that could result from the affiliation in regard to the evolving community healthcare landscape.
One of the potential challenges that could emerge in the community aspect of the agreement is noncompliance. Just because the services are being offered to patients doesn't
necessarily mean that they will utilize them. They may choose to ignore the fact that an agreement is in place and continue to use the ED of TPMC, which could cause even bigger financial strains for TPMC.

It is possible that the agreement between TPMC and The Mayo Clinic could lead to a great opportunity. If the affiliation goes well, TPMC may seek assistance from The Mayo Clinic to tackle other medical challenges they face. However, this would only work if the current affiliation succeeds, as TPMC would need to evaluate the performance metrics of the existing program to determine the potential for a new one.
