Chart Note: Systemic Lupus Erythematosus This 29-year-old Asian female presents with a chief complaint of extreme fatigue along with general muscle aches and joint pain of the elbows, wrists, hands, and knees. She also mentions a chronic low-grade fever and night sweats. She has a family history of systemic lupus erythematosus and rheumatoid arthritis. Vital signs: T 99.1°F, P 80, R 14, BP 144/88. Heart and lung sounds are normal but she has dyspnea with deep inspiration and expiration. There is a malar rash of both cheeks with a “butterfly” appearance. There is bilateral axillary lymphadenitis with tenderness. No evidence of splenomegaly on palpation. Pitting edema of 1+ in both feet. Labs are ordered including UA, CBC, ANA, RF, and ESR. Order full-body CT scan with and without contrast. Prescribe NSAID for joint pain and fever and topical steroid cream for rash.
