Use a medical dictionary such as Taber’s Cyclopedic Medical Dictionary, the appendices of the textbook, or other resources to define each term. Chart Note: Pneumonia This 50-year-old female presents to the emergency department with fever, cough, dyspnea, and signs of pneumonia. On expiration, fine wheezes could be heard, which was not unusual since she suffers from asthma. She said that pulmonary function tests have shown reduced lung capacity, but that her asthma was well controlled. Using a stethoscope, crackles could be heard. A chest x-ray was administered and confirmed a diagnosis of pneumonia. A sputum culture was ordered to determine the causative pathogen and arterial blood gases were ordered to assess the extent of respiratory impairment. Patient was admitted to the hospital for treatment. Treatment plan included intravenous antibiotics stat, to be modified pending results of sputum culture.
