What is medical non-adherence? This is when a patient does not take the medicine that is prescribed to them. As a Pediatrician who has been following a 9 y.o. patient diagnosed with Diabetes Mellitus Type 1 approximately 1 year ago, who is now not having stable blood glucose readings and their Hemoglobin A1c (HbA1c) is elevated and the patient was hospitalized last week for ketoacidosis, some bias has been raised. One example I can think of is how the medication copayment may have increased. Through the study done by Matthew L Maciejewski, Chris L Bryson, Virginia Wang, Mark Perkins and Chuan-Fen Liu titled “Potential bias in medication adherence studies of prevalent users”, they explored this bias. The results of the study concluded that “Medication adherence trends differed when different medication use
criteria were applied. Contrary to our expectations, similar adherence trends were observed when the same medication use criteria were applied at index dates when no copayment changes occurred” (
Maciejewski, 2013). Also, thinking back to the social determinants of health
we learned in class, in addition to the example above, some factors like socioeconomic status come in to play here. The last bias I could think of was if the child’s parents had no way to pay for their child’s medicine and if they even had the proper access to healthcare and benefits. Not
all patients may have the capabilities to extend their supply of medicine or be able to provide enough money to cover for it in the first place. Additionally, someone who I may recommend my patient to would be the American Diabetes Association or the American Medical Association. The organizations would be able to provide different strategies to implement with the child, support the child needs and will help to “To prevent and cure diabetes and to improve the lives of all people affected by diabetes” (About Us, n.d.).
