C.H. is a 67-year-old male patient admitted to the hospital with a primary diagnosis of
S/P stroke leading to hemiplegia and dysphagia along with extensive cardiac conditions like
hypertension, heart failure, hypertension, cardiomyopathy, and coronary artery disease (CAD).
Relating to this, In Canada, the stroke population has surged to 878,000, with a quarter of them
under 65, and the risk of stroke increases significantly after the age of 55 which describes that
these heart conditions are closely related to stroke (D’Aguiar,2022). Healthcare access and
disparities also account for the associated factors for stroke in a population. The issue to address
in this scenario is creating a comprehensive cardiac rehabilitation program that caters to C.H.'s
unique medical history and challenges, while also accounting for the interplay between his
cardiac conditions, stroke recovery, dysphagia, medication management, and emotional well-
being. According to Sabih et. al., (2023), secondary prevention reduces the risk of vascular
diseases and includes risk factors like smoking, diabetes, atherosclerosis, and carotid artery
disease. Adherence to recommended medications, blood pressure monitoring, modifiable risk
factor reduction, and recovering independence through therapy are all strategies (Lewis, 2018).
Nurses play a crucial role in patient care by following adult learning principles for health
education.
Nurses must provide immediate management, monitor vital signs, and address specific
issues like swallowing difficulties. The scenario describes nurses' role in hypertension
management, healthy diet, facilitating exercise therapy, and monitoring dietary adherence
(Lindsay et al., 2008). Moreover, collaboration with physicians, occupational and speech
therapists, social workers, and community nurses is crucial to promote the overall health of the
patient and thus ensure comprehensive care.
