Avoiding medicine mistakes
Medications errors can be caused by improper teaching to patients as well as thoughtless
mistakes made by healthcare providers. These errors can cause life threatening adverse effects.
Nurses especially play a critical role in ensuring adverse drug effects do not harm the patient
(Portera 2011). The Joint Commission drafted a Speak Up brochure on how to help avoid
mistakes with your medicine. The brochure was created September 11
th
2015. It is given to the
general public to inform them about how they can help to avoid committing errors with their use
of medicine that they take.
Summary of Brochure
The brochure explains who is responsible for your medications what you should know
about them and why it is important that you know about all the medicine you take. According to
the Joint Commission, “Medicine mistakes happen everyday at the doctor’s office, in the hospital
even at home”. Medicine errors can be prevented and the brochure explained what you can do to
prevent making errors with your medicine. A person can prevent medication errors in many
ways; it includes knowing what time they should take their medicines. Taking extra precautions
by asking questions about medicines and having a list of medications, reading the list carefully
making sure it lists everything you are taking.
Evaluation of Brochure
The brochure has an abundance of information. They did well giving simple and feasible
approaches that can help patients to not make mistakes with their medications. This nursing
student thinks the only thing that can be improved on the brochure is telling the patient every
time they go to the Doctor they should bring the list of the medications they are currently taking

and review it with the doctor to make sure the dosing is still appropriate and they are taking as
prescribed. This nursing student was also intrigued by this brochure in light of the fact that so
many individuals are making medication errors because of the absence of proper teaching or
simply because patients are afraid to speak up when it comes to their medications. Medication
mistakes are not a joking matter and can cause serious harm to the individual.
The information
on the brochure was exceptionally gainful to individuals understanding it, since it has a
considerable measure of data that can enable them to diminish their danger of making mistakes
with their medications at the hospital and even at home. This nursing student feels that the
information on the brochure can be incorporated when teaching the patient and giving them
instruction about not making medication errors.
The information and guidelines with in the brochure can help keep the patient from
making mistakes with medicine. There is a considerable measure of research at the present time
about risk for medication errors and this brochure has a simple to the point way for helping
individuals in the most straightforward method for disclosing how to lessen the danger making
medication errors. The brochure and other research have a similar path on the best way to keep
individuals from making medication errors. As per other research, patients should have a general
knowledge of any medication allergies they have. The distinction between side effects and
allergies are of significant importance (Schoenly, 2017).
Overall, using various strategies to educate patients and proper communication between
patients and their healthcare providers can reduce the risk for patient related medication errors.
This nursing student believes this brochure can be beneficial to everyone, however the patient
age group that would most benefit is the elderly. The brochure will increase the patient’s safety
when taking medication, because everyone reading the brochure should be able to easily

understand what the brochure is about.
The brochure clarifies information that the patient needs
to know in the least difficult and briefest way that the patient can comprehend. All information
the patient needs is on the brochure and it will help them to decrease or even prevent the risk of
making medication errors.
Conclusion
In conclusion, a large number of the patient population do not understand the importance
of knowing about their medications. Educating every patient about their medications and
knowing that your patient understands can prevent patient related medication errors. It is also
imperative to encourage patients to communicate with healthcare providers. Communication is
key to minimize medication errors.
