Week #6: Scenario Worksheet Instructions: Please respond to the questions at the end of each scenario. Your answer should be limited to the available space on the page where the scenario is written (11pt Calibri). Scenario 1: You are playing soccer and your team has just finished their third game of the day. The day has been very hot with temperatures in the 90s with no cloud coverage. Suddenly, one of your teammates collapses. They seem to not be fully awake and do not respond to questions you ask them. Their breathing is rapid and shallow, and they are unable to get up from the ground. Their skin is very red and hot to the touch but without any moisture. You want to help your teammate. (1) What are the initial steps you should take in this emergency? (2) Describe in detail how you would decide what your plan of care would be. The initial steps that should be taken in this emergency would be: 1. Take your teammate to an area that is cool and away from direct sun exposure. Remove all needless clothes and lay the person down on his or her side to reveal as much skin as possible. 2. Heatstroke happens as the body's temperature is unable to control itself and begins to increase, frequently to temperatures of 40°C (104°F) or higher. Below are symptoms of a quickly developing heatstroke: Loss of consciousness. Possible seizure. Fast breathing or shortness of breath. Increased heart rate (40°C or higher) Vomiting or diarrhea. Not sweating when having a high temperature. Skin that appears to be hot, dry, and red. Not responsive. Feeling confused and possibly anxious or aggressive. Scenario 1 Continued There is an indoor facility close to the field and you have water and ice available to you. There are no higher trained medical personnel currently at this event. (1) What steps would you take to care for your teammate? Describe the steps in detail. (2) When would you know to stop caring for your teammate? The steps that would be taken to care for the teammate would be: 1. Take your teammate to an area that is cool and away from direct sunlight. 2. Remove all needless clothes and lay the person down on his or her side to reveal as much skin as possible. 3. Cool the person's whole body with cold water sponging or spraying, then use a fan to help decrease the person's body temperature. 4. Seizures, unconsciousness for more than a few seconds, and mild to extreme trouble breathing are all symptoms of quickly escalating heatstroke. Apply ice packs to the person's armpits and the back of their neck. To treat a high body temperature induced by heatstroke, do not give aspirin or acetaminophen. Because of the body's reaction to heatstroke, these medications can cause problems. 1
5. Give your teammate fluids [1 L (32 fl oz) to 2 L (64 fl oz) over 1 to 2 hours] if they are awake and alert enough to swallow. It's likely that you'll be asked to assist. Make sure the person is sitting up straight enough to prevent choking. Many people suffering from heatstroke are unconscious and cannot easily be given water to drink. When the person begins to feel better and the signs and symptoms of heat stroke / sun stroke return to normal, dial 911 immediately. Scenario 2: While at a family cookout, your 10-year-old cousin walks to the dessert table to grab a cookie. They walk over to you and starts to eat the cookie, and you notice that the cookie has nuts in it. A couple minutes later your cousin starts to complain of a really bad stomach cramp and some nausea. You want to help. (1) What are the initial steps you would take in this situation? (2) How would you determine what your care would be? The initial steps that should be taken in this situation would be: 1. Call 9-1-1 or the designated emergency number immediately. 2. Try to keep your cousin calm during the situation. 3. Help your cousin lie down, flat on their back. 4. Elevate their feet (about 12 inches) and provide a blanket. 5. If they are vomiting or bleeding, assist them to lay on their side. 6. Make sure their clothing is not restricting any area of the body. Symptoms of tree nut allergies include: Nausea or vomiting. Cramps or stomach pains. Diarrhea. Skin develops red bumps (hives) Itchy skin and rash. Runny nose or congestion. Shortness of breath. Tingling sensation of the mouth and throat. Throat begins to tighten and makes swallowing difficult. Scenario 2 Continued: As you are caring for your cousin, they begin to have difficulty breathing and their lips and tongue are starting to swell. They have a medical bracelet that says they has severe allergies to tree nuts. (1) What steps would you take to give care to your cousin? Describe them in detail. (2) How would you know when to stop caring for your cousin? The steps to take when caring for your cousin would be: 1. Begin performing CPR. Help your cousin lay in a flat position with their head turned to the side to allow the secretion to exit and prevent an airway blockage. Begin CPR by administering 30 chest compressions and 2 mouth-to-mouth breaths. 2
2. Inject epinephrine (auto injector) and call 9-1-1 or the designated emergency number as soon as possible. If they're having trouble breathing, don't give them oral drugs, something to drink, or raise their head. 3. Try to keep your cousin calm during the situation. 4. Elevate their feet (about 12 inches) and provide a blanket. 5. If they are vomiting or bleeding, assist them to lay on their side. 6. Make sure their clothing is not restricting any area of the body. After performing these steps, wait patiently for emergency services to arrive. When your cousin is comfortable enough to sit and walk after vomiting or epinephrine (auto injector), ask them to relax and call in a doctor for a checkup and more advice. Assist the survivor in knowing what to eat and what not to eat. 3
