To code radiology procedures correctly, a coder must have the following
information: the name of the procedure and its medical necessity, how many
views will be used, the findings of the study and the provider’s conclusion,
the provider’s signature on the procedure, and any diagnostic studies done
that help confirm the correct coding (MedicalCodingCert, 2015, 1:04). The
exact procedure, including views used, and why it’s being done are essential
for correct coding as these require individual codes and modifiers used (for
example, initial versus subsequent exam, are multiple procedures being
done, number of views, etc).
Radiology procedures usually include both a technical component and a
professional component. Falling under the technical component are the
following: the procedure performed and the equipment, supplies, personnel,
and cost used to perform it (Dustman, 2023). Modifier TC indicates that only
the technical component was provided.
Under the professional component are the following: the supervision of the
procedure and physician services in interpreting the results). In cases where
both a technical and professional component are present, modifier 26 may
be reported to identify the professional component (Smith, 2021, p. 200).
A global service refers to cases in which when the physician “bears the
expense of equipment, supplies, etc., and provides supervision and/or
prepares the report” (Dustman, 2023). For example, this may apply in an
office setting where the physician is also the practice owner. For global
services, the modifiers 26 or TC are not needed.
