1. How is the NP in the documentary a Disruptive Innovator? The NP is a Disruptive Innovator because she is providing more convenient care that is less expensive. The services and products for these home patients are convenient as well. Although these services are less expensive, the patients the NP sees are faced with challenges that limit their assess to necessary treatment. Most of these patient's health insurance does not cover certain cost of care that is needed at home. 2. Discuss at least one relationship between your personal beliefs and values with this ethical dilemma/barrier. I think that home bound patients should be able to receive treatment weather they are home or in the hospital. The 30-year-old patient with muscular dystrophy was faced with options where he would have to choose care he did not want in order to receive what he need. The first example was when he was told he would have to go to the hospital to be put on the ventilator when the time came because Medicaid would not cover one for home use. After that, it was determined that hospice care would be beneficial to him, but he would have to be a DNR in order receive hospital care. A tough ethical decision that has been made was when the NP was completely honest about his poor prognosis.No matter what medication he took it would not make his heart function any better it would on give him some relief. Having to be completely honest when the patient and family while they are going
through sadness and emotions associated with end of life changed is really challenging. 3. Describe at least one mechanism to overcome the barrier discussed. To overcome the barrier discussed, more patient advocacy and health care reforms would need to be adjusted. The np in this video is a great patient advocate. She does make necessary calls and decisions to help patients as much as possible. Unfortunately, something’s are out of her control. Patients receiving care at home should not have to worry about things not being covered under their insurance. 4. Examine the potential cost and benefits to this barrier. The benefits to this barrier would be the patient would not have made the hard decision of being an DNR or going to the hospital to be put on the vent. Benefits for the decision to choose hospice care includes the patient being home with his family comfortable while going through his transition.
