Deliverable 3 Description (might be a review & recommendations) B. Phase II, once approved, will include the following elements and shall be completed by DATE. Description of deliverables 1. Is a high level view of the project deliverables and is related to work packages. 2. Broadly defines the scope of the project, Uses nouns to describe each box. 3. Is not a hierarchical structure. In other words each box is not a subordinate to the previous box. Under Funding, for example, we identify different sources of funding but each source is not related to the other funding sources. Deliverables (how the project progress will be measured) During the life of this project, the team will: 1. Define Requirements 2. Prepare a draft report on (use of Measure/T2) 3. Complete Phase-I B-(as described in RFP). 4. Conduct CLIENT platform training and admin set up. 5. Submit Phase-I Report complete with Phase–II proposal to the Guidance Team. Action Plan (how the work will be completed) In order to ensure the project is completed on time, the team will adhere to the attached Action Plan dated DATE. Stakeholder line managers will support the project by allowing operational staff to assist the project team, as outlined in the action plan. 3. Content will be delivered in Yardstick’s Excel template for import Constraints 1. Client is unavailable during the month of July 2. Fixed launch date 3. Government regulations and legislations 4. Anything else that is already exists and is a roadblock that cannot be adjusted Potential Risks 1. SMEs do not complete their allotted question assignments by DATE. 2. Mitigation Strategies 1. The project manager is responsible for smooth delivery of (site) and therefore must work very closely with the CLIENT’s project department to ensure the NAME project is completed on time. 2. The PM must provide bi-weekly progress reports to the Guidance Team. 3. Late completion is not an option. Parameters and Principles (budget, schedule, and how we plan to work together) Parameters (Budget, start and end dates, major milestones, critical decision points/gates etc.) 1. Project shall not exceed the budget: Phase-I = $xx,xxxx 2. Phase-I of the project will commence on DATE and shall be completed by DATE. 3. Management of the day-to-day activities of the project shall be based on Yardstick’s standard project management methodology. Principles (how the project team plans to work together) 1. The project team will develop a set of ground rules, make decisions through consensus, and make decisions based on facts and not opinion. 2. If the team fails to reach consensus, the issues will be escalated to the Guidance Team. 3. Stakeholders will be given an opportunity to provide input before decisions are finalized. 4. Team members will be responsible to ensure their departments are kept informed of the team’s progress by maintaining ongoing communications with the manager of the respective work unit.

The Executive will provide final approval to proceed through the Executive Sponsor. Executive Sponsor The Executive Sponsor is a senior manager who is interested in successful completion of the project and must be on the Guidance Team. The Executive Sponsor will act as the liaison between the Guidance Team and the Executive (Client). Guidance Team Guidance Team will provide the project team a draft project charter, approve the project plan (charter and the action plan), accept or reject team recommendations and provide the necessary support/resources to the project team throughout the life of the project. Project Team Leader The project team leader, in collaboration with the project facilitator, will: 1. Provide leadership to the team, and 2. Maintain ongoing communication with the guidance team and the stakeholders. Project Facilitator The facilitator supports the project team leader and the team by providing facilitation skills, and tools & techniques to assist the team by gathering data, facilitating workshops, and assisting the team make decisions through consensus. Stakeholders Anyone/unit/organization that is impacted by the outcomes of this project is referred to as a stakeholder.
