Dent 134 Case Study Assignment Student Name: ________________________ Case Study Name/#: ____________________ Overview Using the case study assigned to you develop an oral hygiene education plan. You will need to decide what oral hygiene instructions/education is most appropriate for this client. Keep in mind the goal is to provide individualized client centred care for each client and what you teach and educate them on should be based on the assessment information you collect on them. So be sure to really pay attention to the case study details 😊 . You should consider what you see clinically, for example is there any inflammation evident on the patient and if so where is it located. How effective is this patient with plaque removal? Are there areas they are missing? This information should help drive your decision on what oral self-care devices you select and or methods of self-care technique (s) you teach them, for example brushing method, adjunct oral-self care devices or a therapeutic agent recommendation ect. You also need to consider their goal/perceived need as you should try to connect this with your oral hygiene education, as often this can help to motivate them. In addition to this, you should factor in actual needs based on your assessment findings and address these in your oral hygiene discussion. For example, is there lots of plaque present, gingivitis evident, decalcification present/active decay, is diet increasing caries risk etc. Does you client understand what is causing these oral conditions? If we don’t point out what we see and these important connections they might not understand the cause of disease in their mouth, and as a result will be less likely to by into behavior changes. Use the guided questions and the OHI teaching principles you learned in this course (the hand guide) to help you pull out important information you will be discussing with your patient. Case Study Guided Questions Write down answers to the following case study questions. So, basically write down what you would say to an actual patient if they were sitting in your dental chair. PRE-ASSESSMENT & GOALS Assess client’s current homecare habits- write these down in the space below (frequency of brushing, flossing or lack of, use of other self-care devices or therapeutics). Consider why they may or may not be doing something as well as you might need to speak to this with your oral hygiene education:
Assess Toothpaste - Is this patient using fluoride toothpaste? (should address why this is important either way with each client you see so write down a sentence on what you would say to this client to include with your OHI: Assess client’s goal or concerns (remember you need to try to include this in your OHI to this client as often this is a motivator for them. Write down your client’s goal or chief complain/concern below: Assess diet - based on your case study do you need to address diet and caries risk with this particular client? If yes write down what you will say to them and when doing so consider how you will relate this to make it client centered and meaningful. If you feel this is not necessary write down rational why that is: Oral Assessment finding – Refer back to your case study for this and write key finding down here. This is important to factor into your oral hygiene instruction. For example, if you notice gingivitis present you would point this out to the patient (tell them the location of it) and educate them on what causes this. (make sure you write this assessment down and include in your OHI). Do they even know they are suffering from gingivitis? Do they know what is causing it? – lets assume not for the case study. It is your job to connect the dots, be sure to make it client centred. Don’t give them a text book answer, make it means something to them. If there is plaque present in those areas point that out and relate this connection to the patient’s tissue health as an example. Assessment finding will help you determine what techniques/methods, devices and therapeutics you might recommend to your client as well. You should also factor in oral assessment finding when determining selection of fluoride and your patient’s caries risk. Write down all relevant assessment findings here:
ASSESS BRUSHING AND FLUORIDE TOOTHPASTE Assess Brushing Method: Determine (write it down) what brushing method you should introduce/review with this patient. When you assess and teach brushing methods to your patient don’t forget to consider where they are not effectively removing plaque biofilm as this should drive your overall discussion and focus with your client’s brushing. Don’t forget to Show them and discuss areas they are missing to help them be more effective in removing plaque in these areas. (make sure write down all the steps on how you should be teaching your client how to brush). Hint – how do you know how they are brushing? How would you assess this, and assume you are not evaluating this with a plaque index at this point in the process. Brushing method recommended (name it and give instructions to method your teaching): What is your teaching approach with this (we discussed this in class under teaching steps)? How do you know that your patient has learned what you have taught them? Areas client is missing and should focus on (based on your clinical assessment findings): Assess Flossing Method: Is this client flossing? What flossing method would be best to teach this client? Consider if they could benefit from the use of a flossing aid? Base on this client’s oral finding (consider size of embrasures space) determine if flossing is appropriate or consider if there a better interproximal cleaning aids that could be more beneficial to them? If so write this down and explain why? Flossing method selected and why: Or if you would recommend a different or adjunct interproximal cleaning device with this client: Other Aids/Devices: Based on your assessment of this client consider one adjunct self-care device that you could introduce to them (don’t repeat your recommendation twice here):

ORAL ASSESSMENT Based on your oral assessment is there gingivitis or inflammation present on this client? If yes where is it located: Write down exactly what you would say to this client about the association (role) that plaque is playing with any gingivitis and/or perio that you see in their mouth. Make sure you make it client centered (relevant to this client not a textbook answer 😊 ). Write down exactly what you would say to this client in regards to the role that is evident in their mouth relates to caries risk or any other relevant finding you see. Consider if this client is low, moderate or high and tell them why. Be specific on how you would explain the role of plaque and caries. Set a hypothetical goal for this client. Remember is should not only be relevant to their need, but realistic, achievable and measurable: Write down agreed upon goal: Document your case study discussion using the guidelines for proper OHI documentation that has been provided for you.
