Please provide a screenshot of encoder usage for your answer. How to do a Screen Shot in a 3M encoder: Pyelonephritis is a UTI where one or both of the kidneys become infected and inflamed. The tabular list instructs the coder to use an additional code to identify infectious agent. The bacteria E. Coli is documented in the summary and lab reports should be listed as secondary code. Coding guideline B5 states that if you have a definitive diagnosis and the conditions are part of the disease they do not need to be coded separately. Flank pain is a common symptom associated with acute pyelonephritis. Coding guideline III A previous conditions states that family history can be coded if the conditions impact the current care or would change the plan of treatment for the patient.
