Safety Issues/Concerns
Workers (not steel connectors) were standing underneath a steel beam that was being lifted by a
crane.
They had to ensure that the crane had enough counterbalance to safely lift the roof modules.
Extreme heat during the day can pose a risk to workers through dehydration and heat stroke.
Active spotters were used during crane lifts to ensure safe practices.
All workers at dangerous heights are using some sort of fall protection.
All workers on site are wearing appropriate PPE.
There was a safety meeting demonstrating fall protection including lanyards and suspension
trauma straps.
They had safety meetings on zero injury practices.
The jobsite had safety signage everywhere needed on the job site, including maps, restricted
areas, and hazard warnings.
Communication, especially using signals, is used by cranes and workers for safety.
Focus on establishing safe access such as egress, ladders, crossovers, and bridges was reiterated
during a toolbox talk.
Safe lifting practices were reiterated during a toolbox talk.
Ensuring clear pathways to prevent slips, trips, and falls was emphasized during a toolbox talk.
Supervisor ensured that workers knew what hazards would be present when transitioning to
new tasks.
During a company safety talk, the supervisor made workers aware of the hazards present while
assembling a new crane, and to avoid the area for safety.
Workers were lead through the bend and stretch to increase productivity and help reduce
injuries.
Weekly OAC meetings are held, and safety is discussed during these meetings.
Weekly AJHA meetings are held to discuss job hazards and risk mitigation.
Monthly meetings with RTC to discuss delivery and road closures for safe delivery of materials.
Drone technology is used to scan the jobsite to deliver data used by safety professionals to
develop emergency response planning and means of egress.
Scaffolding was used inside for construction and must be properly assembled to ensure safe
working.
Assembly platforms were installed and bolted into place for safe assembly of the roof.
Toe boards were used on assembly platforms to prevent injury from falling objects.
Appropriate guardrail systems were used on the assembly platforms to ensure safety while
working on the platforms.
Workers on belay with climbing ropes have or must have fall arrest systems in place.
Strict regulations were put in place for which workers were allowed on site to help prevent the
spread of the Covid-19 virus.
Safety signage about Covid-19 was put in place at the entrances of the jobsite to help educate
workers.
Entryways and other surfaces were disinfected to help prevent the spread of Covid-19.
Horizontal lifelines were used to safely navigate through the cores during the construction of the
roof.
Restricted zones requiring 100% tie off were put in place during roof construction.
Double lanyards were required while navigating the horizontal lifeline system to ensure safe
movement between cables.
Workers were trained in safety practices before entering the 100% tie off zone before beginning
work.
Safety lines and nets were used while assembling the roof system.
Pool floaties were used to ensure that the lines did not flip up in the wind.
Workers wore appropriate hand protection when handling steel lines to help prevent hand
injury.
Workers all appeared to be wearing pants and appropriate footwear while on the jobsite.
Walkie talkies were used to ensure good communication, which helps ensure safety on the site.
While unrolling/rolling and installing the roof cables, workers are at risk of severe injury from
getting body parts pinched/caught in the cable and rollers.
Signs were placed all around the arena reminding workers of the importance of safety. Examples
are “Thank you for making safe choices today!” and “Safety isn’t just about you, it's about
family”.
Designated areas and leading edges were properly marked with warning lines/orange plastic
netting.
Safety cones were used to mark hazard zones while tensioning the roof cables.
Face coverings were required on the jobsite during the brunt of the pandemic to help prevent
the spread of Covid-19.
Anchor points on stanchions between horizontal safety lines were painted bright red so workers
would know where transition points were for their lanyards.
Critical lifts by cranes must be done on low wind days to prevent any incidents caused by high
winds, and to ensure that cranes are lifting within their safe working limits.
