A1. Describe the issues with clinical documentation in the hybrid record: Because a
hybrid record system is a mixture of both electronic and paper records, those documents are
unable to be found in the same location making retrieval of records more difficult. A facility must
determine which types of documents will be paper and which electronic and where they can be
found as well as a system for locating each item. The facility must also have a policy in place for
adding addendums. The trouble with addendums in a hybrid system is that while electronic
addendums are straightforward, there is no way to trace the source of a change on paper.
A2. Explain how a fully digital record will address and solve the issues caused by the
hybrid record: By having a fully digital record, all of the patient documentation will be easier to
find because they will all be in the same location and it eliminates the need to have staff whose
sole purpose is to find records and the facility can reallocate the job function of the personnel to
another area. Another concern digital records eliminate is whether staff have the complete record
needed for a subpoena which can be hard to put together in a hybrid record where everything is
in separate locations.
A3. Describe how the roles of existing staff members in the HIM department will change
with the transition from a hybrid record to a fully digital record: With the implementation of a
digital system, HIM staff will no longer be needed solely to retrieve records. Instead, they can
branch out into other areas of administration. Front end staff can be trained to retrieve records, or
the billing team can take on that responsibility. The staff who previously retrieved records can
take on more of the role of data steward and ensure that those requesting records are following
HIPAA regulations and only requesting what is necessary to do their job.

A4. Explain the revised roles and responsibilities of two HIM professionals in the SDLC
for this implementation project: One role that is important to the project is the health information
manager that oversees the entire project. They are the ones that will set the goals and analyze the
needs of the facility. They help in the design of the program to make sure it meets regulation
requirements and will oversee the implementation of the system. Finally, the manager will
continue to monitor the system and ensure everything runs smoothly.
Another important role is those that analyze clinical data for patient care and run
statistical reports. Data analysts would have a deep understanding of what is needed in the digital
system and know the environment needed to help plan out the project. They would be able to
provide feedback during the analysis phase and for the design phase. The analysts would assist in
development and can test out the system so that it can be implemented with the hopes of having
fewer errors. The final phase-which is maintenance will take the involvement of all staff that use
the system. Continuous feedback and communication are necessary to understand where updates
may be needed.
