Corrective Action Report
Issue Identified: Breach by a New Supplier - Incorrect Storage and Transportation of Frozen Items
Date of Incident: 18th October 2023
Description of the Issue:
During the review of the Food Safety Program logs for the week of 15th October 2023 - 21st October
2023, it was discovered that a new supplier had failed to adhere to the required storage and
transportation standards for two frozen food items in a delivery made on 18th October 2023. The breach
constitutes a serious violation of our food safety procedures and could pose a risk to the quality and
safety of our products.
Corrective Action Taken:
1.
Immediate Isolation of Affected Items:
Upon discovering the breach, the two frozen items in question were immediately
isolated and removed from our inventory to prevent any potential contamination.
2.
Communication with Supplier:
We promptly contacted the supplier in question to notify them of the breach and to
demand an explanation for the deviation from agreed-upon storage and transportation
standards.
3.
Internal Investigation:
Our internal investigation team conducted a thorough review of the incident to assess
the extent of the issue and identify any other potential areas of non-compliance.
4.
Product Quality Assessment:
The isolated frozen items were subject to a quality assessment to determine if there was
any compromise in product quality. This assessment confirmed that the items were
unsuitable for use.
5.
Review of Supplier Relationship:
In light of this incident, our supplier relationship with the non-compliant supplier will be
reviewed. This includes an evaluation of their adherence to our food safety standards
and the potential for continued collaboration.
6.
Staff Training and Awareness:
A training session will be conducted to ensure that all staff members are aware of the
proper protocols for receiving and handling deliveries. This training will emphasize the
importance of immediately identifying and reporting any deviations from standard
procedures.
Communication:
An official email communication was sent to the supplier outlining the breach, its consequences,
and our demand for corrective actions. A copy of this email has been recorded in our
communication log for reference.
Internal staff were informed through a department-wide communication detailing the incident,
corrective actions taken, and the importance of vigilance in monitoring supplier deliveries.
Preventive Measures:
This incident serves as a reminder of the significance of upholding food safety standards. To prevent
similar occurrences in the future, we will implement the following preventive measures:
1.
Enhanced Supplier Oversight: We will implement stricter monitoring of supplier compliance with
our standards.
2.
Staff Training: Regular and updated training sessions will be conducted for staff to ensure they
are well-informed about our food safety procedures.
3.
Periodic Audits: We will schedule periodic supplier audits to verify their adherence to our safety
standards.
4.
Improved Communication: We will maintain open and clear lines of communication with all
suppliers to address any issues promptly.
