IV.1. Validate assignment of diagnostic and procedural
codes and groupings in accordance with official
guidelines.
IV.1. RM Determine diagnosis and procedure codes
according to official guidelines.
IV.3. RM Evaluate compliance with regulatory
requirements and reimbursement methodologies
Your organization is considering using computer-assisted coding (CAC). You create the test case
that follows to see how different applications will code the scenario to help you reach a purchase
decision.
Mr. Reynolds is a 57-year-old white male admitted with chest pain. He developed this pain
while shoveling snow outside his home. He ranks it an 8 out of 10 on the pain scale.
He has no
arm or jaw pain.
He has hyperlipidemia and hypertension and is
on medication for both
conditions.
Last year, he suffered an acute myocardial infarction and
was treated with two
coronary stents.
An EKG was performed, and cardiac enzymes drawn. All results were normal. The patient
exhibited the pain upon movement and palpation at the chondrosternal joint and a final diagnosis
of sprain was determined.
Codes assigned by CAC:
R07.9 -
Admit diagnosis is the chest pain
I21.3 -
R68.84
S23.421A-
Principal diagnosis is the chondrosternal joint sprain
I10- Hypertension
E78.5 –
Hyperlipidemia
Do not add the myocardial infarction code (I21.3) as the MI is not current but could use
the code for old MI I25.2
Do not code the jaw pain, R68.84, as it was stated that the patient did not have this
Could assign Z95.5 for the coronary stents.
Could assign Z79.899 for the long-term use of medication.
1.
Determine any areas of inaccuracy in the CAC identification.
The areas of inaccuracy in the computer assisted coding identification includes:
o
R68:84 it had been verified that the patient had no arm or jaw pain.
o
I21.3 should not have been assigned since the patient had been
o
diagnosis with myocardial infraction a year prior and had been treated.
2.
Recommend corrections to the code assignment.
Recommend corrections:
o
The code for the two coronary stents that was used to treat the patient for myocardial
infraction.
o
A code for long term medication for the patient's hypertension and hyperlipidemia
condition.
