Module 2 Assignment 1. Describe the objectives of failure mode and effects analysis, fault tree analysis, and value analysis. Apply one of the techniques to a project, computer assignment, or writing assignment you have recently completed. FMEA – anticipate failures and prevent them from occurring. FTA – visually identify defects and solutions. VA – map out all the functions of a product to determine what is most valuable. FMEA on tightening a bolt might look like the following: Failure Mode: Cause of Failure: Effect of Failure: Corrective Action: Too tight Rotating too much Strip out threads Use torque wrench Too loose Not rotating enough Bolt falls out Use torque wrench Cross threaded Not hand starting Bolt doesn’t hold components together Hand start bolt 2.   I would recommend Yourizon based on data below
3.  How can the results of queuing analysis be used by a decision maker for making decisions? By understanding arrival rate and service time queuing analysis can provide details to the best waiting line structures. If the queue analysis shows that customers cannot be serviced faster than they arrive decision makers will have to address the problem. 4.  The Minute Stop Market has one pump for gasoline, which can service 10 customers per hour (Poisson distribution).  Cars arrive at the pump at a rate of 5 per hour (Poisson distributed). a.  Determine the average queue length, the average time a car is in the system, and the average time a car must wait. Queue length: (5*5)/10(10-5) = .5 cars Average time in system: 1/(10-5) = 0.2 hours Average wait: 5/10(10-5) = 0.1 hours b.  If, during the period from 4:00 pm to 5:00 pm, the arrival rate increases to 12 cars per hour, what will be the effect on the average queue length? 12*12 /10( 10-12 ) During that time the queue length would increase since the arrival rate is higher than the service rate 5.  List and explain the six factors that affect the decision to outsource.  Explain the sourcing continuum. Cost, Capacity, Quality, Speed, Reliability, Expertise. The sourcing continuum is a representation of the company and supplier involvement required when making a decision on outsourcing. 6.  Construct a process flowchart of a process with which you are familiar. Identify bottlenecks, potential failure points, and opportunities for improvement.   Below is a process flowchart I made to help supervisors and maintenance troubleshoot a particular failure mode where I work.
