. I chose Olympic Medical Center for several reasons that are very personal to me. First, I have been a Registered Nurse working for Olympic Medical Center for 10 years. This is the hospital that I did my clinical preceptor in as a nursing student, and they hired me as a new graduate. The second reason I chose OMC is that I also live in the community that this hospital-based organization serves. I have the unique perspective of an employee that gets to serve its community by working here and as a patient that utilizes Olympic Medical Centers services. 3. Olympic Medical Center is a hospital based medical center that includes an ambulatory clinic component. Olympic Medical Center hospital is considered a community rural hospital short term acute care facility that is government controlled. It has a total of 78 staffed beds and has an overall patient experience rating of 3/5 stars. Olympic Medical Centers hospital-based services include Emergency Services, OB, Short Stay, Surgical Services, Medical Surgical Unit, Intensive Care Unit and Single bed Certification. Olympic Medical Centers ambulatory care services include Orthopedics, Cardiac rehabilitation, Physical
Therapy, Pediatrics, Primary Care, Urology, and Walk-in services.

4. Olympic Medical Centers overall star rating is 3/5 stars. According to Care Compare the over overall rating is based on how well a hospital performs across 5 different areas of quality. The first rating is Mortality. This measure examines death rates in the 30 days following a patient’s hospitalization. The second rating is Safety of Care. This examines potentially preventable injury and complications related to care received while in the hospital. The third star is Readmission to the hospital following a hospitalization. The fourth start is rating the Patient Experience. These results are based off the HCAHPS Survey. The HCAHPS Survey is a standardized publicly reported survey of patients’ perspective of hospital care. The fifth and final star is Timely and Effective Care. This star looks at how often and how quickly hospitals provide care that research shows get the

Olympic Medical Center is enrolled in the DNV accreditation program that provides guidance directly related to regulatory requirements for healthcare organizations such as US Government’s Centers for Medicare and Medicaid (CMS) and helps support healthcare organizations to follow best practices

5. One organizational ethic that Olympic Medical Center provides is that it is a rural community hospital that strives to provide care to a diverse patient population that is safe,
effective, patient-centered, timely, efficient, and equitable. Their mission statement is “Working Together to Provide Excellence in Healthcare”. (OlympicMedical.org n.d) A second example of organizational ethics that Olympic Medical Center operates by is the support provided to staff that work for this hospital-based organization. Olympic Medical Center provides a comprehensive benefits package that includes 401K match contributions, full medical and dental coverage as well as access to flexible spending accounts to cover the cost of unforeseen medical bills or childcare expenses. But most notably is the free assistance the organization offers its employees. The Employee Assistance Plan or EAP is a free service that is accessible to all employees regardless of time worked at the organization. It provides free access to legal services, counseling for employees and all their dependents as well as financial counseling and budgeting classes. By offering these services to employees it helps with retention and reduction of compassion fatigue and burnout. 6
. One of the nurse’s main roles in fiscal responsibility is to always act as a good steward. One of Olympic Medical Centers core values is stewardship. It is expected that nurses will carefully and responsibly protect and manage the resources entrusted to the hospital. This would include double checking orders before performing any type of care to ensure that the proper supplies are being utilized. This will help not only with patient safety but can help reduce waste of incorrect or unnecessary supplies being used. Another way the nurse can help maintain fiscal responsibility is patient education. Educating a patient on how to safely administer a medication at home and what signs and symptoms to look for.
By doing this it can cut down on the rate of repeated unnecessary hospital visits or possible readmissions. 7. One healthcare organizational change that could improve the delivery of value-based care would be to focus on collaborative preventive healthcare. By implementing a strategy to focus on the Primary Care setting and engaging the patient in prevention and prediction of
potential chronic illness. This model will help improve the delivery of the value-based model and help reduce the future cost of healthcare to the patient and the hospital district. B. Olympic Medical Center services the communities located in Sequim and Port Angeles, WA and its surrounding areas which make up Clallam County. Some neighboring communities also utilize the services provided by this hospital district. Another hospital that is comparable to Olympic Medical Center is Valley Medical Center located in Renton, WA approximately 2 hours from Olympic Medical Center. Valley Medical Center is also a government hospital that is owned and governed by its residents. They are a public hospital district that is a community supported government entity that serves its residents and its surrounding population. According to the American Hospital Directory, Valley Medical Center is a short-term acute care facility that has emergency services, a cardiac unit, short stay, OB, surgical services, and a medical surgical unit. According to Teisberg et al, 2020 the definition of value-based healthcare can be defined as the measurable improvement in patient outcomes for the cost to achieve said improvements. Valley Medical Center offers not only acute care services like access to emergency services and surgical services they also offer wound care. Many patients that seek these services do not have access to primary care services and often utilize acute care
facilities like Valley Medical Center as their main management of medical care. By serving this patient population I believe that Valley Medical Center is more aligned with value-based care. 2. The patient star rating of both Olympic Medical Center and Valley Medical Center according to Care Compare is 4 out of 5. Based off the timely and effective care category Valley Medical Center has 1% of all patients in the emergency department left before being seen. Washington state average is 4% and the national average is 3% of all patients leave before being seen. (American Hospital Directory (n.d))
When patients leave the emergency department prior to care received it can have both financial and medicolegal implications for a hospital. For patients it can lead to a delay in care and potentially higher healthcare costs in the future. (Smalley 2021) Due to this lower-than-average number at Valley Medical Center, patients are being seen in a timely and efficient manner as well as being provided equitable patient centered care. 3. A financial quality data source that can inform organizational transformation is CMS Hospital Compare website. CMS website breaks down the different rates of patient care measures of hospitals quality and performance. By utilizing the CMS website, healthcare organizations can implement this data to help improve processes and ways to implement

quality care. A quality data source that can inform organizational transformation for a healthcare institute is the DNV Accreditation program.
