Research and define 'culture of blame' and 'culture
of safety'. Do you consider your current (or past)
workplace to have a culture of blame or a culture of
safety? Support your choice giving specific
examples, if possible.
“A culture of blame refers to an environment where
people, or groups/ people are frequently singled out
and blamed, criticised and fault is apportioned for
mistakes and errors.” (Blame Culture: Definition and
Explanation, n.d.)
Example: when a new nurse cannot get an IV, and
the older nurses are criticizing the new nurse for not
being able to get the vein, and they are making the
new nurse feel insecure/ guilty because def doesn’t
know.
Culture of safety: “A culture of safety culture can be
described as the shared perceptions, beliefs, values,
and attitudes that combine to create a commitment
to safety and a continuous effort to minimize harm.”
(Creating a Culture of Safety for Nurses and Patients,
n.d.)
Example: rather than making the new RN feel guilty
& insecure, the older nurses should give tips on how
to perfect the IV, and teach the RN, that way the
patient gets efficient care in a timely manner.
Use research to identify one way to promote or
enhance a culture of safety in your workplace?
A way I promete safety in my workplace is by
speaking up if one of my patients needs something I
can deal with myself regardless if the older nurses
talk bad about me or not.
How comfortable are you reporting an error or near-
miss in your workplace?
I personally never have had to report an error or a
near-miss in my workplace. However, if I had to I
would feel pretty comfortable because at the end of
the day, it’ll be for the safety of our patients.
Does your workplace provide for anonymous
reporting?
Yes, my workplace does provide anonymous
reporting. All I have to do is log on to the computer
and choose wether the reporting will be anonymous
or not.
