Client Description and Intake Interview
As a case manager, the type of clients, and needs they require, will be wide ranging and
ever changing. You will work with a diverse group of clients that do all have one thing in
common, and that is needing help from someone to guide them in the right direction. With every
client you encounter, it is crucial during the initial intake interview to get as much information as
possible, including a description of the type of service the client needs, to ensure their return.
Today, a client came into my agency seeking services and during the first interaction I will be
getting as much demographic information about him as well to help tailor the services
specifically to their needs.

I remember as I was just unlocking the doors to the agency on one Monday morning, and
there stood a woman looking desperately like she needed some help. She was a Caucasian
woman by the name of Taylor Allen, and she was a mother of two around the age of 24. She was
born on October 11
th
1999 here in Jacksonville, Florida to her parents who ended up putting her
up for adoption at an early age due to suffering from substance abuse. Unfortunately, like that of
her parents, she also developed a substance abuse problem that she was currently struggling with.
Taylor also began to drink heavily along with taking her substance of choice, Xanax.
As Taylor and I began to talk, Taylor’s husband Joe was supporting her throughout her
substance abuse. However, when his wife started mixing Xanax with it, he became quite

concerned. Their two daughters began noticing as their mother did little to hide it, and eventually
Joe left with their two daughters to prevent them from being around it. Now, Taylor is struggling
to find a job, and maintain sobriety to try and fix her estranged marriage. As she concluded
telling me a gist of her situation, she heard of my agency which is how she got here this morning.
I will then begin by asking her a serious of questions in regards to her situation, identifying the
problem and establishing its severity, and discovering her strengths that she can rely on.

When did you first think that you needed to get help with your substance use?

How often will you drink when taking your substance of choice?
3)
How often will you drink while trying to maintain sobriety?
4)
At what age did you begin drinking and using substances?
5)
How much alcohol and substance did you estimate using last?
6)
Do you have a history of trauma including abuse, grief, neglect, or being a victim of
crime?
7)
Have you partaken in any other substance or addictive behaviors that have not yet been
shared with me?
8)
Are there any other mental or physical disorders you currently struggle with?
9)
Does anyone in your life, or living situation, have an issue with substances or alcohol?
10) What led you to come in for assistance with your problems?

Reflection on Biases and Pitfalls
As I begin to work closer with Taylor, personal bias becomes a huge possibility when
keeping track of their progress. Being she is my client who came to me directly, I have an extra
layer in this partnership as I strive to help my client succeed. One of the biggest concerns I have
would be involved with her use of alcohol and other substances while caring for her two
children. Ultimately, a clear mind is needed to properly care for children as they develop and
learn the habits of their superiors. In this case, it would be important to reevaluate my role as a
case manager to overcome this bias. It is crucial that when approaching my client, I enforce the
utmost empathy and positive attitude towards her situation to show that she has someone to
support her. Her success and recovery is important to me being her case manager, therefore
supporting her is the ultimate way to achieve success.
As a case manager, I must put aside any personal biases to make the environment a
positive experience for the client. Understanding that each client, and their situation, is unique to
them can help prevent this bias as well as their background can be drastically different to mine.
In this case, it’s important to realize that her substance abuse and drinking are a part of a disease,
and not just a lifestyle like most portray. This will be another important factor to keep in mind
when progressing with the client to prevent bias throughout treatment.
