Client Needs – Care Plan Template Needs All answers should be based on the case study and care plan, and must address how the care plan meets the family’s needs. For additional prompting questions, see the Milestone Three section of the Final Project Document. Evaluate the extent to which the plan addresses the children’s needs. How do the interventions and referrals in place address the needs? How do the interventions and referrals differ for acute versus ongoing needs? Do they fully address the needs? The needs of the family include behavioral and substance abuse help for Eloni, safety for Kolomalu, counseling for each person individually and as a family, housing and financial stability. The primary intervention of removing Eloni from the home addressed the need for safety of Kolomalu as Eloni was physically harming him. This is an acute solution to the need of safety for Kolomalu as the family is working towards bringing Eloni home, removing Eloni does not address the underlying cause of him attacking Kolomalu. Screening and assessment tools as a secondary intervention address the needs by evaluating each individual and the family to determine the level of trauma experienced, as well as what types of traumas. The assessment and screening tools can also help determine the individuals risks for certain behaviors and actions such as substance use. By finding out information related to the trauma, the appropriate course of counseling can be prescribed and conducted, also, any drug or alcohol use screenings can determine if Eloni is in fact utilizing any substances, and if so, how to go about treatment for that. Assessment and screening tools would be an acute need but can be utilized again later in treatment to re-evaluate the progress made. The tertiary intervention is counseling. Counseling is needed by everyone, as well as the family as a unit. Counseling can address both behavioral and substance use for Eloni, as well as all traumas he has experienced. Kolomalu would benefit from counseling to address the physical attacks from his brother, and Anamalia can benefit from counseling as a parent, as well as to process any potential traumas she has experienced. Family counseling will help the family work together towards goals established and process traumas together. Counseling is an ongoing intervention and can benefit the family for longer periods of time. Discuss the gaps in service of the plan. How do the interventions or referrals not address the children’s needs? What recommendations would you make to address these needs? The needs of employment for Anamalia and financial stability are not addressed through the interventions in the service plan. The stress of their financial situation can be discussed in counseling; however counseling would not provide Anamalia with a job or money. To address the needs of employment and financial stability, I would recommend that Anamalia connect with local temp- agencies and would also provide her with employment websites to look through. She has recently obtained her MA degree, so I would also help her contact local doctors offices or hospitals for employment applications. Evaluate the extent to which the plan addresses the family’s needs. What steps are in place to address the family’s needs? How do they address the needs? How do the steps differ for acute versus ongoing needs? Do they fully address the needs? Are there gaps where needs are not addressed? The primary intervention within the plan addressed the need of safety for Kolomalu as Eloni was removed from the home and he was the one abusing Kolomalu. The family was then referred to this human service professional for counseling and evaluation. The next step would be to conduct assessment and screening tools for the family and determine the traumas, level of traumas, and what they are at risk for. By conducting these assessment and screening tools, appropriate counseling services can be recommended and provided. Counseling services would be a final step for the family as it can help each person work on their individual needs as well as their needs as a family. They can also work on addressing the traumas experienced and how to process those traumas. Most of the needs within the family are ongoing, however the immediate need of safety for Kolomalu can be considered acute for the time being as the family works towards his continued safety around his brother Eloni. This acute need was met by the removal of Eloni from the home. This step would not address the ongoing need for his safety however as the family is working towards Eloni returning to the home. Assessment and Screening tools would address the acute need of determining best counseling strategies to utilize as well as obtaining the information necessary to better treat the family. The ongoing need for counseling,
